Book excerpt: The performance of a CAD system for subtle lesions is generally much lower than their performance for less subtle lesions. The goal of this project is to develop a CAD system using advanced computer vision techniques aiming at improved detection of retrospectively seen cancers on prior mammograms and incorporate the developed CAD system into our current CAD system. During the project years, we have performed the following tasks: (1) collect the data sets of digitized film mammograms for training and testing our CAD system, (2) develop a series of single-view computer vision techniques for mass detection and classification in prior mammograms, (3) reduce FPs by correlation of image information from multiple view mammograms of the same patient, (4) develop a information fusion scheme to combine the new CAD system with the existing CAD system for mass detection, and (5) evaluate the effects of the newly developed CAD scheme with a large data set. We have found that our new computer-vision techniques can significantly improve the performance of the CAD system for mass detection by JAFROC analysis. The significance of this project is that the newly developed CAD system may be able to aid radiologists in detecting breast cancers at an early stage. Since early detection and treatment can reduce breast cancer mortality rate and health care costs, the proposed CAD system will improve the efficacy of mammography for breast cancer screening.

Book excerpt: Architectural distortion is an important and early sign of breast cancer, but because of its subtlety, it is a common cause of false-negative findings on screening mammograms. Screening mammograms obtained prior to the detection of cancer could contain subtle signs of early stages of breast cancer, in particular, architectural distortion. This book presents image processing and pattern recognition techniques to detect architectural distortion in prior mammograms of interval-cancer cases. The methods are based upon Gabor filters, phase portrait analysis, procedures for the analysis of the angular spread of power, fractal analysis, Laws' texture energy measures derived from geometrically transformed regions of interest (ROIs), and Haralick's texture features. With Gabor filters and phase-portrait analysis, 4,224 ROIs were automatically obtained from 106 prior mammograms of 56 interval-cancer cases, including 301 true-positive ROIs related to architectural distortion, and from 52 mammograms of 13 normal cases. For each ROI, the fractal dimension, the entropy of the angular spread of power, 10 Laws' texture energy measures, and Haralick's 14 texture features were computed. The areas under the receiver operating characteristic (ROC) curves obtained using the features selected by stepwise logistic regression and the leave-one-image-out method are 0.77 with the Bayesian classifier, 0.76 with Fisher linear discriminant analysis, and 0.79 with a neural network classifier. Free-response ROC analysis indicated sensitivities of 0.80 and 0.90 at 5.7 and 8.8 false positives (FPs) per image, respectively, with the Bayesian classifier and the leave-one-image-out method. The present study has demonstrated the ability to detect early signs of breast cancer 15 months ahead of the time of clinical diagnosis, on the average, for interval-cancer cases, with a sensitivity of 0.8 at 5.7 FP/image. The presented computer-aided detection techniques, dedicated to accurate detection and localization of architectural distortion, could lead to efficient detection of early and subtle signs of breast cancer at pre-mass-formation stages. Book excerpt: In November 1999, the Institute of Medicine, in consultation with the Commission on Life Sciences, the Commission on Physical Sciences, Mathematics, and Applications, and the Board on Science, Technology and Economic Policy launched a one year study on technologies for early detection of breast cancer. The committee was asked to examine technologies under development for early breast cancer detection, and to scrutinize the process of medical technology development, adoption, and dissemination. The committee is gathering information on these topics for its report in a number of ways, including two public workshops that bring in outside expertise. The first workshop on "Developing Technologies for Early Breast Cancer Detection" was held in Washington DC in February 2000. The content of the presentations at the workshop is summarized here. A second workshop, which will focus on the process of technology development and adoption, will be held in Washington, DC on June 19-20. A formal report on these topics, including conclusions and recommendations, will be prepared by the committee upon completion of the one-year study.

Book excerpt: In June 1998 the Fourth International Workshop on Digital Mammography was held in Nijmegen, The Netherlands, where it was hosted by the department of Radiology of the University Hospital Nijmegen. This series of meetings was initiated at the 1993 SPIE Biomedical Image Processing Conference in San Jose, USA, where a number of sessions were entirely devoted to mammographic image analysis. At very successful subsequent workshops held in York, UK (1994) and Chicago, USA (1996), the scope of the conference was broadened, establishing a platform for presentation and discussion of new developments in digital mammog raphy. Topics that are addressed at these meetings are computer-aided diagnosis, image processing, detector development, system design, observer performance and clinical evaluation. The goal is to bring researchers from universities, breast cancer experts, and engineers together, to exchange information and present new scientific developments in this rapidly evolving field. This book contains all the scientific papers and posters presented at the work shop in Nijmegen. Contributions came from as many as 20 different countries and 190 participants attended the meeting. At a technical exhibit companies demon strated new products and work in progress. Abstracts of all papers were reviewed by members of the scientific committee. Many of the accepted papers had excellent quality, but due to limited space not all of them could be included as full papers in these proceedings. Papers that were rated high by the reviewers are included as long or short papers, others appear as extended abstracts in the last chapter.

It is the most common cancer disease in women worldwide, especially those who are in the age group of 40 to 55 years and one of the leading causes of death from cancer in women. Breast cancer is reported as the second most prevalent cause of the cancerous death in the report of World Health Organization]. Breast cancer is"the second most common cancer worldwide. There were over 2 million new cases reported in 2018. The recent statistics of a study by American Cancer Society gives an alarming message that about 3,30,080 new cases of breast cancer are expected to be diagnosed in women in the U.S. and about 2,550 new cases of invasive breast cancer are expected to be diagnosed in men in 2018. These new cases of breast cancer in women are further classified as 2,66,120 new cases of invasive and 63,960 new cases of non-invasive breast cancer. A man's lifetime risk of breast cancer is about 1 in 1,000 whereas 1 in 8 for women which is about 12.4% chances of having breast cancer over the course of the lifetime . The worldwide cancer statistics report says breast cancer is the third most common cancer in the world in terms of number of new cases . In India, there are around 2.5 million diagnosed with cancer and will lead to 5,56,600 deaths per year. By 2020, India is reported to have over 17.5 lakhs diagnosed patients with breast cancer and near 8.8 lakhs deaths caused by this disease . As reported in literature, most prevailing cancer in India is breast cancer and its explanation is a 27% count in all types of cancer in women. Early detection is an exhausting effort and a nerve center on cancer detection. Imaging modalities and advancements in technology disported a far-reaching role in early detection [4]. Mammography has been extensively recognized as a diagnostic and shielding weapon for an early detection of breast cancer and even up to ten years before detection and diagnosis stage. Mammography can curtail the number of mortality native to breast cancer, particularly among women in an age group of years 40-70. Mammography implies the assimilation of anomalies such as calcifications and masses to detect breast cancer. It makes image analysis as a thrust area of research for an early detection of breast cancer. The enactment of a Computer Aided Diagnosis (CAD) system can revamp the early cancer detection rate and could also share the experts workload [6]. A recent report manifested that the enactment of CAD refined the process of breast cancer detection by 7.62% and this automated or semi-automated gizmo has captivated great considerations [6]. Many Computer Aided Diagnosis systems prospected to assist radiologists to curtail the diagnostic errors effectively by using mammogram and MRI imaging modality of breast mass or calcification [33] [26]. During this project year, we have performed the following tasks: (1) collected databases of digital mammograms (DMs) and digitized film mammograms (DFMs) for development of the CAD systems, (2) conducted a study to compare the percent dense area manually segmented by experienced radiologists on DMs and DFMs, (3) developed new image enhancement techniques and new false-positive reduction methods for mass detection, and conducted studies to compare the accuracy of mass detection by the CAD systems for DMs and DFMs using FROC analysis, (4) developed automated method for nipple detection on mammograms as a basis of multiple image fusion analysis for CAD systems, and (5) compared the accuracy for classification of malignant and benign breast masses using single-view and fused two-view information on mammograms by computer, and evaluated the effects of CAD on experienced radiologists' characterization of malignant and benign breast masses in two-view temporal pairs of mammograms. In summary, we have investigated a number of areas in CAD of mammographic lesions and evaluated the new techniques for both DMs and DFMs. We have made progress in the six tasks proposed in the project. We have found that our new computer-vision techniques and two-view information fusion approach can improve the performance of the CAD systems. We will continue the development of the CAD systems for%Ms and DFMs in the coming years.

Book excerpt: An intelligent Computer-Aided Detection system (CAD) can be very helpful in detecting masses in the breast earlier and faster than typical screening programs. Two such systems are presented, first a system based on Radial Basis neural networks coupled with feature extraction techniques for detecting masses in digital mammograms. Suspicious regions are identified following a run of the trained neural network. Five co-occurrence matrices are constructed at different distances for each mammogram. A number of statistical features are used to train and test the Radial Basis neural network. The second system presented was developed based on linear subtraction and feature extraction techniques to identify asymmetries between left and right breast mammograms. This system is based on the idea that a deviation from the normal architectural symmetry of the right and left breasts could indicate a cancerous mass. The mammograms were first normalized, registered, and then linear subtraction was followed by various feature extraction techniques that were used in order to reduce the number of false positive detections. Computerized detection was evaluated by using Receiver Operating Characteristic analysis (ROC), which measured the overall sensitivity of the presented systems by the area under the curve Az. The results show that both systems could be helpful to the radiologist by serving as a second reader in mammography screening.
