Book excerpt: With one in three American children classified as obese or overweight, efforts to improve the health status of our youngest generation are imperative. Interventions that improve nutritional knowledge and increase physical activity levels are proven to decrease the body mass index of children. Elementary schools are prime settings for nutrition instruction. This project formed a partnership between a school of nursing and an elementary school and developed an afterschool nutrition education program. Student nurses spend their clinical hours providing lessons to elementary students using a research-validated nutrition curriculum. The project achieved a sustainable, and replicable approach to the childhood obesity epidemic and affirmed the feasibility of collaborations between nursing programs and elementary schools to provide innovative clinical experiences that promote the health of children. Most importantly, it will help elementary students and their families develop healthy habits that can reduce their risk of obesity and optimize their health throughout life.

Health problems once seen overwhelmingly in adults, such as type 2 diabetes, cardiovascular disease, and hypertension, are increasingly appearing in youth. Though the health of Americans has improved in many broad areas for decades, increases in obesity could erode these and future improvements. The IOM report Accelerating Progress in Obesity Prevention: Solving the Weight of the Nation recognized the importance of the school environment in addressing the epidemic and recommended making schools a focal point for obesity prevention. The development and implementation of K-12 nutrition benchmarks, guides, or standards (for a discussion of these terms, see the next section of this chapter) would constitute a critical step in achieving this recommendation. National nutrition education curriculum standards could have a variety of benefits, including the following: Improving the consistency and effectiveness of nutrition education in schools; Preparing and training teachers and other education staff to help them provide effective nutrition education; Assisting colleges and universities in the development of courses in nutrition as part of teacher certification and in updating methods courses on how to integrate nutrition education in subject-matter areas in the classroom and in materials; and Establishing a framework for future collaborative efforts and partnerships to improve nutrition education. Nutrition Education in the K-12 Curriculum: The Role of National Standards is a summary of the workshop's presentations and discussions prepared from the workshop transcript and slides. This summary presents recommendations made by individual speakers.

The curriculum was designed to be presented once a week over the course of four 3-hour sessions addressing the following nutrition topics: MyPlate and portion sizes, healthy beverage choices, disease associated with overweight and obesity, physical activity for the whole family, healthy snack preparation, and intuitive eating and the psychological aspect of childhood obesity. The team for the 5-4-3-2-1-0 Steps to Healthy Families childhood obesity prevention program consists of a Registered Dietitian, and psychologist specializing in pediatrics. The Registered Dietitian is responsible for all of the nutrition education, physical activities, and healthy snack preparation and demonstrations, while the psychologist focuses on intuitive eating and the psychological, social, and emotional aspects of dealing with childhood obesity. This nutrition education program was developed to be used in the San Fernando Valley area to help lower the childhood obesity rates in the community of Mission Hills, California. Qualitative observations could be made during implementation of the program, which could lead to modifications and improvements to the curriculum; further research is suggested to collect, analyze, and evaluate the effectiveness of the curriculum in changing knowledge and behavior of participants.

Book excerpt: Childhood obesity remains a major concern in the United States, as 17% of children and adolescents ages 2-18 are obese. Although changes in knowledge alone are not enough to change behavior, most successful nutrition interventions to reduce childhood obesity include nutrition education. The purpose of this study is to understand practices that impact the effectiveness of childhood nutrition education, and to identify expert opinions of essential best practices for childhood nutrition education. A literature review was conducted to identify previously established best practices for childhood nutrition education for 6-12 year olds. Eight sources were identified, and 33 best practices were cited. Ten members of a USDA multi-state research group reviewed the best practices and edits were made according to their feedback. Three practices were added, and four were deleted. In order to understand experts’ opinions about childhood nutrition education, an interview script was developed based on the revised list of best practices. Cognitive interviews were conducted with faculty members of the University of Tennessee, Knoxville Department of Public Health Nutrition, and Extension agents. Three practices were deleted, and eight practices were combined. To ensure clarity, a second round of cognitive interviews were conducted with the same interviewees. Four practices were added during this process. A total of 23 best practices were included in the subsequent interview and ranking process. Four experts received a list of the final 23 best practices and were asked to rank the top ten best practices. Through an interview, experts were asked to provide greater detail about each of the 23 practices, explaining and describing in their own words. Emerging concepts were documented in narrative form. Experts reviewed this document to ensure that the document represented their thoughts accurately. This study found that there is great variability among the literature and experts concerning best practices for childhood nutrition education interventions. In the future, best practices for specific settings and age groups should be established and disseminated in order to improve the effectiveness of childhood nutrition education interventions.

It has been estimated that 43 million pre-school children from birth to the age of 5 years are overweight and obese, while 92 million are at risk of becoming overweight. In Kuwait, a cross-sectional study investigating overweight and obesity rates of 5402 girls and boys aged between 10 -14, reported an overall prevalence of 30.7% and 14.6% overweight and obesity, respectively. Combating this pandemic through intervention programs of single or multiple components were investigated internationally, however, no such interventions were applied in Kuwait. Aims The aim of this study is to assess the effect of structured nutrition education program, hands-on cooking classes and goal setting and behavioral modification techniques/ strategies on: 1) Change in eating habits and behavior of overweight and obese children and their parents; 2) Change in energy and nutrients consumption, and change in weights, BMIs, and waist to hip ratio in overweight and obese children and their parents; 3) Change in lipid profile in overweight and obese children; 4) Change in quality of life for overweight and obese children and their parents; 5) Maintenance of change in eating habits after 3, 6 months in overweight and obese children and their parents; 6) Compare findings from different parameters with findings from a matched control group.Material and methods Fifty overweight and obese girls and boys, aged between 8 -12 years, and their mothers were recruited. A number of anthropometric and biochemical test were conducted at baseline and 6 months after intervention. Both mothers and children were asked to fill out u201cNutrition Knowledge assessment survey, a u201cHealth related Quality of life questionnaireu201d and a u201cFood Attitude and Behavioru201d adapted survey at baseline and after 6 months. Class plans, structured nutrition education classes material and cooking classes themes were developed by certified dietitians at Dasman Diabetes Institute. The intervention was provided in the form of 2 sessions per week for 3 weeks. Interactive nutrition education lectures, behavioral modification strategies and techniques, hands on cooking classes, and exercise sessions were provided by certified dietitians, certified exercise trainers and a professional chef.Similarly, assessment of anthropometric and biochemical test was conducted for 25 participants from the control group at recruitment, and 6 months without the 3 week intervention program. Controls also filled out similar questionnaires, surveys and forms filled out by intervention group following the same time-line.Results Statistical analysis is in process. Paired sample t-tests will assess changes in anthropometrics, biochemical data and change in food nutrition knowledge, quality of life and food attitude and behavior within individuals and between groups. Correlations, repeated measures and logistic regression will be applied after controlling for potential confounders.Discussion Preliminary findings indicate increase in nutrition knowledge and positive attitude towards improvement in eating habits and behavior. We postulate that combining hands on cooking classes with structured nutrition classes and setting behavioral modification strategies may contribute to curbing the prevalence of childhood obesity in Kuwait.

Obesity in children not only affects them during childhood and adolescent, there are health issues that can follow them the rest of their lives. A few examples of health illnesses that are not age specific are heart disease and diabetes. Not only does childhood obesity lead to other preventable health issues it can also lead to low self-esteem and depression. Children do not have always have the right knowledge when it comes to making decisions about food and exercise, this is the reason further education about nutrition and exercise is needed. Educating children and their families about nutrition and exercise will help fight a health epidemic that is preventable. Studies have shown that the easiest and most logical environment to teach health knowledge to children and adolescents is in a school setting. The school environment is best because every child has to attend school; be it public, private or home school. Also, children and adolescents spend about seven to eight hours at school each day and five days out of the week. Implementing this initiative begins by educating the school staff so they will be better equipped to teach the students about health education. By implementing health education into school curriculums, children and families are given the knowledge and skills to combat childhood obesity. The process of evaluation will happen after one school year. If there is too short of time between implementation and evaluation it will cause indiscretions, so a longer period is used to make sure the results are represented accurately.

Book excerpt: The prevalence of childhood obesity has nearly quadrupled from 5.2% to 19.3% in the last 50 years. Many interventions and studies have been proposed and implemented to reduce the prevalence of childhood obesity. The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) is a federally funded program that has helped to reduce the prevalence of childhood obesity in children ages 2- to 4-years old; however, there is still more that can be done to reduce the prevalence of childhood obesity. The purpose of this proposed study is to determine the effects of annual nutrition education reinforcement sessions (ANERS) for mothers on the reduction of children’s BMI-for-age z-scores compared to parents/dietary gatekeepers who do not receive ANERS. Additionally, this proposed study will determine the effects of ANERS on the average number of servings of fruits and vegetables (F&V) that children consume outside of school meals in comparison to the average number of servings of F&V consumed outside of school meals by children whose mothers do not receive ANERS. It is anticipated that 200 mother-child dyads, previously in the WIC program, will participate in this proposed study, with mothers randomized into either the intervention group or the control group. The intervention group will receive annual nutrition education reinforcement sessions with an assigned dietitian after their children’s well-child visit. The control group will not receive annual nutrition education reinforcement sessions. It is anticipated that there will be a significant difference in children’s BMI-for-age z-scores in year two (1.16, ±0.14 and 1.30, ±0.11, respectively; p = .050) and year three (1.07, ±0.18 and 1.28, ±0.12, respectively; p = .025). It is also anticipated that in year three of the study, children in the intervention group will have a significantly greater average consumption of F&V outside of school meals than children in the control group (14.0, ±1.8 and 11.3, ±2.2, respectively; p = .042). The proposed study will be aUse of Nutrition Education Reinforcement to Decrease Childhood Obesity 3 step towards reducing the prevalence of childhood obesity by providing mothers an opportunity to be counseled and to get healthy eating reinforcement annually. Book excerpt: It is feared that children may now have shorter life expectancies than their parents' generation if childhood obesity continues. Nutrition is epigenetic, so if change happens now, future generations will have better outcomes and brighter futures. The purpose of this project is to enhance the nutritional knowledge of children through nutrition education and to understand barriers to improving nutrition education. The objectives of the project are to provide nutrition education to children, demonstrate increasing nutritional knowledge as a result of education, discuss the Nurse Practitioners' role in nutrition education and identify barriers to nutrition education. The nutrition education was provided to two age groups within a daycare setting. The pre-k group's education tools included the "Slow, Go Whoa" curriculum adapted from the U. S. Department of Human Services' "We Can" program. In the school-age group, the tools used for education were adapted from the Myplate.gov website. In each group, knowledge was tested pre- and post-intervention through age-appropriate tests. As a result of the education, the children showed increased nutritional knowledge. The pre-k group showed an increase of 32.2 percent, and the school-age group increased by 69.5 percent. It has become progressively important to focus on primary prevention modalities to slow the progression of chronic disease in all ages. Nurse Practitioners can use nutrition education to make a positive impact on the health of the nation.
