Book excerpt: The purpose of this quality improvement project was to provide a means to effectively assess patients as they are admitted to the hospitals and to effectively communicate a patient’s violent tendencies within the healthcare setting. Workplace violence is a serious problem that affects all healthcare professionals. Although serious assaults and homicides attract more media attention, the majority of workplace violence consists of non-fatal assaults. Nurses, aides, and patient care technicians suffer the most non-fatal assaults resulting in injury. Due to the growing incidence of assault and injury among healthcare workers, some states are calling for additional study on workplace violence. Healthcare organizations are mandated to develop violence prevention programs and greater reporting of incidents. Healthcare organizations have workplace prevention programs in place but still fail to protect healthcare workers from injury. Additional measures are needed to cope up with the increasing incidence of workplace violence specifically related to assaults and injuries caused by patients in healthcare settings. Research shows that the use of a violence risk assessment tool has been proven effective in attempts to prevent workplace violence. This paper will examine patient initiated violence in the workplace, explore the use of a workplace violence risk assessment tool to identify patients with propensity for violence in hospitals, and analyze the implication of the use of a violence risk assessment tool to the nursing practice.

Book excerpt: Nurses working in emergency departments are often exposed to violent acts from their patients. This setting in the hospital is vulnerable to violence with contributing factors including substance use, mental illness, pain, long wait times, and lack of privacy. Staff may use physical restraints to address these behaviors. Predicting behaviors that are precursors to physical violence and early intervention has the potential to reduce violent events, thus reducing the need to physically restrain patients. The purpose of this project was to determine if the implementation of a violence risk assessment tool for patients in emergency departments would decrease the incidence of physical restraint use. The violence risk assessment tool for this project was the Br©ıset Violence Checklist (BVC). In total, a minimum of 128 data sets were collected from patients ages 18 and older who received care in the ED. A quasi-experimental, one-group, pretest, posttest design with use of observation was chosen to complete this project, and an independent sample t-test was used to complete the data analysis. An independent sample t-test was utilized to compare the relationship between the implementation of the BVC and the incidence of physical restraints. Results showed that the incidence of physical restraints, t(34) = -0.26, p = 0.79 were not significantly different between the pre-intervention and post-intervention; therefore, the implementation of the BVC did not decrease the incidence of physical restraints. Results implied that more longitudinal study with a larger sample size may allow further assessment of the effectiveness and offer a more complete conclusion.

Book excerpt: Violence towards healthcare workers in acute care settings, particularly mental health units is increasing nationwide, causing safety and financial concerns for health care organizations. Efforts were made to decrease assaults directed towards healthcare staff on a twenty five bed adult mental health unit in a large academic university hospital. The aim of the project was to decrease assaults by twenty-five percent in six months after implementation of a Violence Risk Assessment (VRA) tool. The ultimate goal was to maintain assault rates below NDNQI. Assaults on the unit for the fourth quarter of 2015 were 2.92, NDNQI 2.05. Assaults with injury were 2.43, NDNQI 0.75. A literature review was performed to ascertain valid and reliable tools in predicting assaultive behavior which resulted in three VRA tools with high predictive value: the Broset Violence Checklist (BVC), Dynamic Appraisal of Situational Aggression (DASA), and Modified Overt Aggression Scale (MOAS). Each tool was piloted on the unit for two weeks followed by a survey to staff regarding their choice of tool based on risk assessment, ease of use, and acceptability (Appendix A). The DASA (Appendix B) was chosen by the staff and approved by the Nursing Steering Committee. Next steps include implementing the DASA tool in the electronic health record with follow up metrics to determine its effectiveness in identifying and decreasing assault potential. Recommendations include implementing EBP interventions to prevent violence for patients identified as assault risk.
