In the United States (US), young adults, especially college students, are mostly affected. HPV vaccine was approved in the US since 2006. They are safe and effective, but coverage remains lower than other vaccines for young adults, implying missed vaccination chances, and a need to improve HPV vaccination promotion strategies. Comics are potentially effective for health education of diverse groups, because they are easily accessible, low-cost, engaging, and unobtrusive. They may help to improve knowledge and beliefs regarding HPV vaccine, increase its uptake, and decrease HPV sequelae.MethodsWe conducted a mixed methods study to examine predictors of intention to complete HPV vaccination among college students in northeast Ohio. We developed an educational HPV/HPV vaccine comic book for college students informed by evidence from current target population-based studies on HPV vaccination, pilot testing, and the Integrated Behavior Model (IBM). We hypothesized that the intervention would improve their HPV vaccine knowledge, beliefs, attitude, perceived norm, personal agency, and intention to complete HPV vaccination within 12 months. Using a quasi-experimental pretest/posttest survey design, we evaluated the effect of the intervention on a diverse population of 18 to 26-year-old male and female students recruited from a college in northeast Ohio in 2017 to review the comic book. ResultsA majority of participants, n=314, males (28%), females (72%) were satisfied with the comic and reported it is an acceptable way to get HPV vaccine information, easy to read, culturally relevant, with good quality information and graphics. Only 29% reported they have completed the recommended three doses of HPV vaccine. Multiple linear regression (n=157) showed age (ß ^=-0.11*); race (ß ^=0.81*); instrumental attitudes (ß ^=0.43*); injunctive norms (ß ^=0.20*); and descriptive norm (ß ^=0.55**) were significant predictors of intention to complete HPV vaccination in the next 12 months [R2=0.47, F (8) =16.12, p

We describe the development and production of a theory-based, audience centered HPV vaccine comic book for East African adolescents. We held three focus groups with East African immigrant mothers in King County, Washington (n=30) to identify themes and suggestions to inform comic book production. We conducted message mapping to bridge focus group themes, theoretical constructs, message objective, and message appeal. Finally, we embedded the theory-based messages into the story script and gathered input from community members on cultural appropriateness. The comic book incorporated emerging themes from the focus groups including mothers’ preference that comic book characters resemble members of their community; resonance with the social connections between the adolescent main character and her social network including adolescent-teacher, adolescent-peers, adolescent-mother, and adolescent-grandmother; integration of emotion through humor; and information needs. The story highlighted information about HPV infection and transmission, vaccine benefits, and risks. The story consisted of three scenes: 1) adolescent HPV vaccine education by a health professional at school; 2) discussion of HPV vaccine beliefs and experiences among a group of adolescent peers; and 3) communication with their mothers about HPV vaccine and intent for vaccination. Through this formative research process we identified suggestions linked to larger established themes regarding HPV vaccine and used them to produce a comic book for East African adolescents. Ongoing testing will assess acceptability and impact of the comic book on HPV vaccine knowledge, vaccination intention, and vaccine uptake.

Human papillomavirus (HPV) is the most prevalent sexually transmitted infection in the United States. College-age males influence acquisition and transmission of HPV due to engagement in high-risk sexual behaviors. HPV vaccination is an efficacious strategy for reducing the burden of HPV-associated morbidity; yet rates of HPV vaccination remain low among college males. The purpose of this study was to operationalize the direct constructs of the theory of planned behavior (TPB) to predict the HPV vaccination behavioral intentions of male undergraduate college students attending a large public southeastern university. Methods. A non-experimental, cross-sectional study design was employed with 256 vaccine-eligible college males. Instrumentation comprised a qualitative elicitation study, face and content validity by a panel of seven experts, readability and comprehensibility by pilot test, stability reliability by test-retest, internal consistency applying Cronbach's alpha, construct validity applying confirmatory factor analysis, and predictive validity applying structural equation modeling. Results. Approximately one third (31.3%) of the sample was unaware of HPV and nearly half (45.3%) of the sample was unaware of the HPV vaccine. The final structural model exhibited acceptable fit of the data (Chi-square test = 129.78; degrees of freedom, df = 70, p = .000; Kline's alternative, KA = 1.854; Goodness-of-fit index, GFI = 0.932; Normed fit index, NFI = .948; Root mean square error of approximation, RMSEA = 0.054). Attitude toward the behavior and subjective norm were significant predictors of behavioral intention, accounting for 58% of the variance in behavioral intention. Perceived behavioral control was found to be a non-significant predictor of behavioral intention. Overall, college males reported low behavioral intentions to get the vaccine (M = 8.52; SD = 5.30). Discussion. A valid and reliable instrument designed to measure constructs from the TPB was developed to predict HPV vaccination intentions of college males. Findings from this study provided an instrument that may be applied in the design and evaluation of TPB-based interventions to promote HPV vaccination among undergraduate college males. Future research may examine possible mediators and moderators of TPB constructs to fully operationalize the theoretical framework.

We evaluated the impact on participating adolescents' HPV and HPV vaccine knowledge, attitudes and vaccination intentions. Methods: The culture-specific comic book intervention was delivered to unvaccinated EAA adolescents who participated in ethno-linguistic-specific dinners with their mothers in Washington (8 dinners in Somali, 2 in Amharic) between 2017-2018. English-speaking adolescents aged 14-17 were assigned to complete a self-administered survey either before or after reading the comic book. Surveys were identical and assessed knowledge, barriers, communication with parents, and intention. Responses between adolescents completing the pre- vs post-test were compared with chi-squared tests. We used generalized estimating equation (GEE) models to estimate relative risks (RRs) and 95% confidence intervals (CIs) to measure the magnitude of the impact of the intervention on each construct. We also stratified the GEE models by gender to enumerate gender-specific intervention effects and tested for the presence of interaction between the participants' gender and the assigned survey group (e.g., pre- or post-intervention). Results: Among 136 (Pre-test=64, Post-test=72) participants, 90% were Somali and 54% female. Adolescents assigned to the post-test were more likely to correctly answer questions on HPV/HPV-vaccine knowledge than those assigned to the pre-test (correct responses ranged from 27%-67% on the pre-test vs. 66%-90% post-test).
