Book excerpt: The overall objective of this proposal is to fully automate and optimize the performance of a 3-D dedicated emission mammotomography system for enhanced semi-automated clinical testing. A retrospective study of 103 clinical MRI uncompressed breast scans was conducted to create surface renderings of the uncompressed breasts and analyze how to adapt existing acquisition orbits for varying breast shapes. Laser ribbon ranging sensors and associated hardware to fully automate the radius of rotation were acquired and tested. An observer based 3D contrast-detail study was performed in an effort to evaluate the limits of object detectability for the dedicated CZT-based SPECT mammotomography imaging system under various imaging conditions. Other aspects of the training program were also initiated including clinical shadowing in the mammography, nuclear medicine, and breast oncology divisions at Duke University.

Book excerpt: The overall objective of this research is to optimize the development of a combined dual-modality single photon emission computed tomography (SPECT) and x-ray computed mammotomography (CmT) system for the earlier detection and staging of breast cancer, improving surgical biopsy guidance, and the monitoring of patient therapy response. It is believed that co-registered acquisition of emission (nuclear) and transmission (x-ray) data using both 3D imaging modalities in a common field of view may aid to accurately localize the radioactive uptake of a tumor in the emission image by using anatomical structure from the transmission image. In this second year, the radiation dose absorbed by the uncompressed breast during a regular tomographic scan using is extensively quantified using the developed tomographic system. New concept designs for the next generation 3D breast CmT system are also developed, in addition to new tomographic trajectories designed to overcome limitations in the ability of the developed CmT system to view the chest wall of the prone patient. In addition to the technical work, other aspects of the training program were also completed, including site visits to a major medical imaging company, the presentation of the research at scientific conferences, and the ongoing submission of a manuscript to a major peer reviewed journal. Finally, the doctoral thesis arising from this research was written and successfully defended by the author.

Book excerpt: The overall goal of this work is to develop, implement, and evaluate novel three-dimensional acquisition orbits used in conjunction with a dedicated emission mammotomography system, to be used as a potential secondary diagnostic tool. This novel nuclear medicine based molecular imaging technique utilizes camera trajectories that sample a pendant, uncompressed breast, which are evaluated with respect to various breast shapes and sizes. Phantom measurements have demonstrated that lesions near the chest wall can be visualized for both large and small sized breasts, and furthermore, that axillary imaging is possible even with minimal system tilt. The use of additional bed shielding, simulated here with a lead sheet, was also investigated. Preliminary results suggest the lead shield may hinder lesion detectability. Future work includes the addition of a patient bed, which is expected to further complicate positioning, and which may or may not include lead lining pending further studies.

Patient bed optimization has also been performed as well as system/bed positioning and tilt angles for optimal chest wall imaging and patient comfort. The outcome of this project is a fully functioning 3D dedicated breast imaging system ready for initial patient studies. In addition, the training provided by this grant has allowed the PI to continue on with further research and development of this system through SBIR and North Carolina state funding with the goal of commercialization.
