But despite concerted methodological efforts, there has simply not been the degree and breadth of innovation that Porter envisaged. Patients’ voices still often remain obscured. This has resulted in part from assumptions about the limitations of archives, many of which are formed of institutional records written from the perspective of health professionals. Patient voices in Britain repositions patient experiences at the centre of healthcare history, using new types of sources and reading familiar sources in new ways. Focusing on military medicine, Poor Law medicine, disability, psychiatry and sexual health, this collection encourages historians to tackle the ethical challenges of using archival material and to think more carefully about how their work might speak to persistent health inequalities and challenges in health-service delivery.

Book excerpt: The distinguished historian of medicine Gerald Grob analyzes the post-World War II policy shift that moved many severely mentally ill patients from large state hospitals to nursing homes, families, and subsidized hotel rooms--and also, most disastrously, to the streets. On the eve of the war, public mental hospitals were the chief element in the American mental health system. Responsible for providing both treatment and care and supported by major portions of state budgets, they employed more than two-thirds of the members of the American Psychiatric Association and cared for nearly 98 percent of all institutionalized patients. This study shows how the consensus for such a program vanished, creating social problems that tragically intensified the sometimes unavoidable devastation of mental illness. Examining changes in mental health care between 1940 and 1970, Grob shows that community psychiatric and psychological services grew rapidly, while new treatments enabled many patients to lead normal lives. Acute services for the severely ill were expanded, and public hospitals, relieved of caring for large numbers of chronic or aged patients, developed into more active treatment centers. But since the main goal of the new policies was to serve a broad population, many of the most seriously ill were set adrift without even the basic necessities of life. By revealing the sources of the euphemistically designated policy of "community care," Grob points to sorely needed alternatives. Originally published in 1991. Founded in January 1912, the League promoted the intensification of German militarism and the cultivation of German nationalism. As the last and second largest of the patriotic societies to emerge after 1890, the League led the campaign for army expansion in 1912 and 1913, and against the growing influence of socialism and pacifism within Germany. Attempting to harness popular and nationalist sentiment against the government's foreign and domestic policies by preying on Germans' fears of defeat and socialism, the League contributed to the polarization of German society and aggravated the international tensions which culminated in the Great War. Coetzee combines an analysis of the League's principal personalities and policies with an exploration of the inner workings of local and regional branches, arguing that rather than having served solely as a barometer of populist nationalist sentiment, the League also reflected the machinations of men of education and prominence who believed that an unresponsive German government had stifled their own careers, dealt ineffectually with the prospect of domestic unrest, and squandered the nation's military superiority over its European rivals.
