Book excerpt: Excerpt from Detroit Medical Journal, 1934, Vol. 3 The importance of an abundance of fresh air and muscular exercise for chil dren cannot be too much impressed on parents as preventive measures; it is use less to modify the diet and dose the child with cod-liver oil, if, at the same time, it is allowed to sit all day long in a close room or crowded school-room. The school-room must be well ventilated, the school hours not too long, and long even ing studies. Must not be allowed. There must be plenty of out-door exercise in gymnastics, or at least with hoop, skip ping - rope or shuttlecock and battledore. The great mischief is that girls give up their exercise too early in life; almost before puberty comes upon them they consider it unwoman-ly to exercise in this manner; but, as thev become too old for their childish games, they must be en couraged to continue to exercise in the open air by a reasonable amount of hock ey, tennis, cricket, cycling or riding, along with their brothers, to whom this exercise is equally a necessity. Indeed, to be successful in our object we must definitely prescribe the amount of food and drink, the number of hours to be allotted to lessons. To play and exercise. And to sleep. Since the adoption of the mcgraw rubber ligature in surgery, one of the chief oh stacles to a finished technique in its use has been the fact that the ligature always made the orifice in the tissues much too large. The thickness of the ligature itself, even when under tension, and the fact that it had to be doubled at the eye of the needle were the two chief factors in this trouble. The style of needle which we represent, the design of Dr. Angus mclean, of De troit, Mich successfully overcomes the difficulty. It is so simple that its advan tages must be self-evident, and yet it perfects the technique in the use of rubber ligatures. A slot in the eye provides for the easy introduction of the ligature into the butt of the needle; there is no threading necessary. When the ligature is intro ducea into the eye, much after the manner of the introduction of thread into the blind man's needle, it is cut off near the eye and a ferule which closely fits the needle is pushed snugly over the ligature and the eye, securely fastening it. In this way the thickness of the needle at the eye end is increased only by the thickness of the ferule, which is inconsiderable, and the surgeon has an 0p portunity for using a rubber ligature without appreciable increase of diameter at its point of entrance into the needle - something which we believe has not heretofore been accomplished in work with ligatures of this type. He should, however, endeavor to make an accurate diagnosis and will be expected by the friends to give a prognosis. For this reason he should make effort to know the character of the work done by different specialists. Not all of those who claim special aptitude possess it. Observation will show that some who talk aseptic principles do not observe them. Specialists will also show different grades of workmanship. It is a question of high art, the products of every one will differ in actual value from those of every other one, and in relation to so sacred an object as the conservation of life and the restoration of health it behooves the medical adviser to know He assumes a great responsibility in recommending an operator and should be paid for this knowledge by the patient, who reaps the benefit. In making the selection he should be guided by the answer to the question: To whom would you refer your wife or mother or sister under similar condi tions.
