Even so, high fertility, defined as five or more births per woman over the reproductive career, characterizes 33 countries. Twenty-nine of these countries are in Sub-Saharan Africa. High fertility poses health risks for children and their mothers, detracts from human capital investment, slows economic growth, and exacerbates environmental threats. These and other consequences of high fertility are reviewed in the first half of this paper. Recognizing these detrimental consequences motivates two inter-related questions that are addressed in the second half of the paper: Why does high fertility persist? And what can be done about it? The high-fertility countries lag in many development indicators, as reflected for example in their rate of progress toward achievement of the Millennium Development Goals (MDGs). These countries have also received less development assistance for population and reproductive health than countries more advanced in their transitions to lower fertility, and the assistance they did receive increased only marginally from 1995 to 2007, a period during which commitments to both health and HIV/AIDS rose substantially.

In most of these countries, the total fertility rate is now below the level that implies a stable population in the long run. This has led to concerns among economists, policymakers, and the wider public about the economic consequences of low fertility and population decline. In this contribution, we aim to i) describe the main determinants of low fertility in high-income countries, ii) assess its potential economic consequences, iii) discuss adjustment mechanisms for individuals and economies, iv) propose a simple economic framework to analyze the long-run economic impact of low fertility, and v) draw lessons for economic policymakers to react appropriately. While the economic challenges of low fertility are substantial, a thoughtful and consistent policy response can mitigate most of the adverse consequences.

Available in PDF, EPUB and Kindle. Book excerpt: High desired fertility is an important factor contributing to the population explosion in sub-Saharan Africa. On a broad sample of 910 respondents from the rural areas of Uganda this paper assesses the impact of health risks, economic contributions from children, traditional community institutions and unequal position of women on desired fertility levels. The paper further scrutinizes how these determinants are affected by education. The results show that fear of diseases and involvement in traditional clan institutions increase desired number of children. Interestingly, these effects can be remarkably mitigated through education that improves the individual health prevention as well as reduces the influence of clans. Economic incentives for having children seem to be less significant than other factors. In addition, a very significant difference in desired fertility between men and women emerges, nevertheless education leads both to reduction and convergence of their desired fertility levels. All these findings suggest that education stimulates a complex change in fertility preferences and underline the importance of education as efficient tool for reducing rapid population growth.

Before 1960, there were substantial improvements in life expectancy, but fertility declines were very rare. Few people used modern contraceptives, and couples had large families. Since 1960, however, fertility rates have fallen in virtually every major geographic region of the world, for almost all political, social, and economic groups. What factors are responsible for the sharp decline in fertility? What role do child survival programs or family programs play in fertility declines? Casual observation suggests that a decline in infant and child mortality is the most important cause, but there is surprisingly little hard evidence for this conclusion. The papers in this volume explore the theoretical, methodological, and empirical dimensions of the fertility-mortality relationship. It includes several detailed case studies based on contemporary data from developing countries and on historical data from Europe and the United States.

The marked declines in fertility seen in some developing nations have been accompanied by slowing population growth, which in turn provided a window of opportunity for rapid economic growth. For many sub-Saharan African nations, this window has not yet opened because fertility rates have not declined as rapidly there as elsewhere. Fertility rates in many sub-Saharan African countries are high: the total rate for the region is estimated to be 5.1 births per woman, and rates that had begun to decline in many countries in the region have stalled. High rates of fertility in these countries are likely to contribute to continued rapid population growth: the United Nations projects that the region's population will increase by 1.2 billion by 2050, the highest growth among the regions for which there are projections. In June 2015, the Committee on Population organized a workshop to explore fertility trends and the factors that have influenced them. The workshop committee was asked to explore history and trends related to fertility, proximate determinants and other influences, the status and impact of family planning programs, and prospects for further reducing fertility rates. This study will help donors, researchers, and policy makers better understand the factors that may explain the slow pace of fertility decline in this region, and develop methods to improve family planning in sub-Saharan Africa.
