Much of the international cancer literature covers the nature and challenges of the disease as seen in high-income regions of the world, thereby giving a skewed view of the global cancer challenges. As the low- and middle-income regions of the world transition from communicable to non communicable disease patterns, there is a need for a corresponding paradigm shift, with increased emphasis on what the world needs to know about non communicable diseases, including cancer, where the disease is hitherto poorly documented. The main goal of the proposed book is to contribute to this outcomes.

It is both a teaching text for students and a resource for researchers and practitioners engaged in the design and scaling of technologies for low-resource communities. The scope is broad, ranging from the development of mobile applications for low-literacy users to hardware and software solutions for providing electricity and water in remote settings. It is also highly interdisciplinary, drawing on methods and theory from the social sciences as well as engineering and the natural sciences. The opening section reviews the history of “technology-for-development” research, and presents a framework that formalizes this body of work and begins its transformation into an academic discipline. It identifies common challenges in development and explains the book’s iterative approach of “innovation, implementation, evaluation, adaptation.” Each of the next six thematic sections focuses on a different sector: energy and environment; market performance; education and labor; water, sanitation and health; digital governance; and connectivity. These thematic sections contain case studies from landmark research that directly integrates engineering innovation with technically rigorous methods from the social sciences. Each case study describes the design, evaluation, and/or scaling of a technology in the field and follows a single form, with common elements and discussion questions, to create continuity and pedagogical consistency. Together, they highlight successful solutions to development challenges, while also analyzing the rarely discussed failures. The book concludes by reiterating the core principles of development engineering illustrated in the case studies, highlighting common challenges that engineers and scientists will face in designing technology interventions that sustainably accelerate economic development. Book excerpt: Telemedicine networks to support healthcare workers in resource-limited settings (often for humanitarian purposes) have evolved over the last decade or so in a largely autonomous way. Communication between them has been informal and relatively limited in scope. This situation could be improved by developing a comprehensive approach to the collection and dissemination of information. A recent review identified seven telemedicine networks, each of which had been in operation for at least five years and which provided store-and-forward telemedicine services to doctors in low- and middle-income countries. These networks provide clinically useful services and improved healthcare access. However, like much of telemedicine, the formal evidence for their cost-effectiveness remains weak. Topics of current research interest therefore include the cost-effectiveness of telemedicine in resource-limited settings. Outcomes data (and methods for gathering it) such as patient quality of life following a telemedicine episode, the knowledge-gain of healthcare staff involved in telemedicine, and staff recruitment and retention in rural areas are also of interest. Finally, there is little published information about the performance of these telemedicine networks (and methods for measuring it), about how best to manage them, and about how to share resources between them. A collection of articles reporting the current evidence supporting the use of telemedicine in resource-limited settings would build the evidence base and should provide a focus for future research. It would also serve to raise the profile of this potentially important research field.

This global burden starkly illustrates the inequality between the developed and the developing world. While the majority of people living in developed countries receive timely treatment, those living in developing countries are not as fortunate and their survival rates are much lower--not only due to delays in diagnosis, but also to a lack of personnel, a paucity of treatment facilities, and the unavailability of many medications. Routine screening--a mainstay in the developed world--could greatly increase the likelihood of identifying individuals with early stage cancers and thus reduce the number of people who present with advanced disease. This book represents a critical addition to the literature of global health studies. Focusing on cervical, breast, and oral cancers, these case studies highlight innovative strategies in cancer screening in a diverse array of developing countries. The authors discuss common issues and share how obstacles--medical, economic, legal, social, and psychological--were addressed or overcome in specific settings.
