En effet, de la qualité de ce lien va dépendre l'efficacité des soins prodigués et l'amélioration de l'acceptation des propositions thérapeutiques. Pourtant la psychologie médicale, définie par l'application des connaissances psychologiques au service de l'étude du lien entre le praticien et son patient, reste à ce jour insuffisamment enseignée aux chirurgiens-dentistes. Ce constat est corroboré par l'analyse statistique de l'étude que nous avons menée auprès des praticiens de la SOMME. Néanmoins, des pistes d'amélioration pédagogiques existent. La mise en place de Travaux Pratiques de simulation humaine avec des jeux de rôles entre étudiants permettrait de mieux les préparer à la compétence relationnelle durant leur formation préclinique. Par ailleurs, des grilles d'auto ou hétéro-évaluation des compétences relationnelles acquises par les étudiants en Odontologie au cours de leur formation clinique pourraient être utilisées, de manière simple et reproductible. Enfin les enseignants formés à la psychologie médicale deviendraient des tuteurs pédagogiques et évalueraient les étudiants dans leur prise en charge relationnelle des patients en situation clinique réelle.

Si le chirurgien-dentiste établit un diagnostic précoce d'exclusion d'une pathologie bucco-dentaire, il n'a pas toujours conscience que l'accueil bienveillant de la plainte douloureuse est primordial pour le futur. Le praticien est souvent désarmé de ne pouvoir proposer une solution médicotechnique efficace et a souvent l'impression d'être inutile auprès de ces patients. Ces douleurs nécessitent une prise en charge biopsychosociale, quelque fois associée à un travail psychothérapeutique. Le praticien doit donc résister à toute envie de réaliser un acte technique invasif pour privilégier l'écoute du patient et de son vécu douloureux. Reconnaitre la réalité de cette douleur permet d'instaurer une relation thérapeutique, une relation authentique et de qualité entre patient et praticien qui permettra d'éviter l'errance médicale et toute iatrogénie dans la prise en charge. L'objectif de ce travail est de mettre en évidence comment la relation thérapeutique entre le chirurgien-dentiste et le patient douloureux constitue un premier levier, comment elle conditionne l'acceptation du patient et son engagement dans une prise en charge multidisciplinaire. Le patient renoncera alors à une recherche incessante de solutions médicales pour envisager la douleur sous d'autres prismes : relationnel, social, psychologique et spirituel. Le chirurgien-dentiste a donc sa place dans ce processus en abandonnant sa posture d'expert exclusivement centrée sur le somatique, pour une posture humaniste et d'ouverture. Prendre le temps d'installer une relation de qualité avec le patient atteint de douleurs oro-faciales chroniques est donc une étape fondamentale avant de l'adresser à une Structure Douleur qui poursuivra la prise en charge.

The Analyst's Reveries finds increasing veneration for the analyst’s use of their reveries, while revealing important differences amongst post-Bionians in how reverie is defined and used clinically. Fred Busch ponders if it has been fully recognized that some post-Bionions suggest a new, radical paradigm for what is curative in psychoanalysis. After searching for the roots of the analyst’s use of reverie in Bion’s work and questioning whether in this regard Bion was a Bionian, Busch carefully examines the work of some post-Bionians and finds both convincing ways to think about the usefulness and limitations of the analyst’s use of reverie. He explores questions including: From what part of the mind does a reverie emerge? How does its provenance inform its transformative possibilities? Do we over-generalize in conceptualizing what is unrepresented, with the corresponding problem of false positives? Do dreams equal understanding and what about the generalizability of the co-created reverie? Busch concludes that it is primarily through the analyst’s own associations that the reverie’s potential is revealed, which further helps the analyst distinguish it from many other possibilities, including the analyst’s countertransference. He believes in the importance of converting reveries into verbal interpretations, a controversial point amongst post-Bionians. Busch ends with the difficult task of classifying the analyst’s reveries based on their degree of representation. The Analyst's Reveries will be of great interest to psychoanalysts and psychoanalytic psychotherapists.
