Book excerpt: In 1977, the current editors contributed a review article on behavioral group therapy to a volume of Hersen, Miller, and Eisler's Progress in Behavior Modi fication series (1977). At that time we noted that, despite the advantages to both clinicians and clients of conducting behavioral treatments in groups, clinical developments and research in this area were still at a relatively rudimen tary level. The majority of studies in the behavioral group therapy literature we reviewed reported the direct transfer of an individual behavior therapy pro cedure, such as systematic desensitization, to a group of clients with homoge neous problems, such as snake phobia or test anxiety. Groups were used in many studies merely to generate sufficient numbers of subjects to allow various types of interventions to be compared, rather than to examine group process variables per se. Only a limited amount of attention had been given to whether these group interaction variables (such as group discussion, sharing ideas and feelings, and mutual feedback and reinforcement) might enhance individually oriented procedures applied in a group. The 8 years since this original chapter was written have seen a significant growth in both the breadth and depth of clinical research and work in the behavioral group therapy field. This growth was documented in part in a three volume series on behavioral group therapy by the current editors (Upper & Ross, 1979, 1980, 1981).

Because breathing continues on its own, without our awareness, it does not necessarily mean that it is always functioning for optimum mental and physical health. The opposite is true often. The problem with breathing is that it seems so easy and natural that we rarely give it a second thought. We breathe: we inhale, we exhale. What could be simpler? But behind that simple act lies a process that affects us profoundly. It affects the way we think and feel, the quality of what we create, and how we function in our daily life. Breathing affects our psychological and physiological states, while our psychological states affect the pattern of our breathing. For example, when anxious, we tend to hold our breath and speak at the end of inspiration in a high-pitched voice. Depressed people tend to sigh and speak at the end of expiration in a low-toned voice. A child having a temper tantrum holds his or her breath until blue in the face. Hyperven tilation causes not only anxiety but also such a variety of symptoms that patients can go from one specialty department to another until a wise clinician spots the abnormal breathing pattern and the patient is successfully trained to shift from maladaptive to normal breathing behavior.

Over the last decade, a growing body of research has significantly strengthened understanding of healthy MEB development and the factors that influence it, as well as how it can be fostered. Yet, the United States has not taken full advantage of this growing knowledge base. Ten years later, the nation still is not effectively mitigating risks for poor MEB health outcomes; these risks remain prevalent, and available data show no significant reductions in their prevalence. Fostering Healthy Mental, Emotional, and Behavioral Development in Children and Youth: A National Agenda examines the gap between current research and achievable national goals for the next ten years. This report identifies the complexities of childhood influences and highlights the need for a tailored approach when implementing new policies and practices. This report provides a framework for a cohesive, multidisciplinary national approach to improving MEB health.

I have continued to maintain my practice according to the same ethical, professional, and business assumptions discussed in this book. One of the reasons for my writing this book is that, at the time I was beginning my practice, there was no one text that helped me begin or to explain what "nuts and bolts" issues I needed to consider. As my practice has grown, I continued to see a need for a resource text to help the beginning therapist get started. I decided to describe the assump tions, methods, and issues that I have used so as to present a discussion of timely issues relevant to the practice of behavior therapy.

Book excerpt: Throughout history, people have invented many different ways to inflict direct and deliberate physical injury on themselves -- without an intent to die. Even today, the concept and practice of self-injury is sanctioned by some cultures, although condemned by most. This insightful work fills a gap in the literature on pathologic self-injury. The phenomenon of people physically hurting themselves is heterogeneous in nature, disturbing in its impact on the self and others, frightening in its blatant maladaptiveness, and often indicative of serious developmental disturbances, breaks with reality, or deficits in the regulation of affects, aggressive impulses, or self states. Further complicating our understanding is the large and diverse scope of psychiatric conditions, such as pervasive developmental disorders, Tourette's syndrome, and psychosis, in which these behaviors occur. This volume presents a comprehensive nosology of self-injurious behaviors, classifying them as stereotypic, major, compulsive, and impulsive (with greater emphasis on the last two categories because they are the most commonly seen). The chapter on stereotypic self-injurious behaviors (highly repetitive, monotonous behaviors usually devoid of meaning, such as head-banging) focuses on the neurochemical systems underlying the various forms of stereotypic movement disorders with self-injurious behaviors, typically seen in patients with mental retardation and autism, and discusses their psychopharmacological management. The chapter on psychotic, or major, self-injurious behaviors (severe, life-threatening behaviors, such as castration) presents a multidimensional approach to evaluating and treating patients with psychosis and self-injurious behaviors, including the neuroanatomy and neurobiology of sensory information processing as background for its discussion of neurobiological studies and psychopharmacological treatments. Chapters on the neurobiology of and psychopharmacology and psychotherapies for compulsive self-injurious behaviors (repetitive, ritualistic behaviors, such as trichotillomania [hair-pulling]) offer much-needed biological research and the first empirical treatment studies on compulsive self-injurious behaviors, and argue that a distinction can indeed be made between compulsive and impulsive self-injurious behaviors. Chapters on the neurobiology, psychopharmacology, and dialectic behavior and psychodynamic theory and treatment of impulsive self-injurious behaviors (habitual, chronic behaviors, such as skin picking) supplement the few neurobiological studies measuring impulsivity, aggression, dissociation, and suicide and detail the efficacy of various medications and psychotherapies. An eminently practical guide with exhaustive references to the latest data and research findings, this concise volume contains clinical material and therapeutic interventions that can be used right away by clinicians to better understand and treat patients with these complex and disturbing behaviors.

It definitely is an amalgam of our interests in social and clinical psychology. It represents our commitment to basic theoretical and empirical inquiry blended with the applications of ideas and methods to understanding attribution in more naturalistic set tings, and as it unfolds in the lives of different kinds of people coping with diverse problems of living. The book represents a commitment also to the breadth of approach to attribution questions epitomized by Fritz Heider's uniquely creative mind and work in pioneering the area. To us, the attribu tional approach is not a sacrosanct school of thought on the human condi tion.
