Book excerpt: Judgments about student nurse clinical competency is the responsibility of the nurse educators who evaluate students' clinical performance. Misjudgments of that performance can have negative consequences for all involved in the process, that is, students and their families, the nursing profession, and the public. In order to evaluate the effectiveness of the performance measurement system used by Regents College, information was collected on how examiners score a Clinical Performance in Nursing Examination (CPNE). During the summer of 1995, examiners were asked to watch a videotaped simulation of a student nurse completing a sample of the critical behaviors tested in the actual CPNE, and to score these as pass or fail. The scoring was completed using observation of the student nurse performance and completion of a behavioral checklist. The sample consisted of 188 examiners who administer the CPNE throughout the United States. The results of the study were that examiners scored the simulated CPNE with a high degree of consistency (interrater agreement) and with a high degree of individual accuracy. In addition, a study of selected faculty characteristics found no relationship between those characteristics and the individual accuracy score. Of the possible 52 critical elements to be scored, examiners achieved a 90% or better agreement for 43 of these elements. Ten of the 52 elements were scored incorrectly and 7 of these errors were attributed to the examiners, while the remaining 3 were attributed to the videotaped simulation. The highest accuracy score to be achieved was 100%, and for the sample the scores ranged from 53.12% to 100%, with a mean of 88.84%. Correlation coefficients for score and selected examiner characteristics found no statistically significant relationship between these variables. At the end of the study it is concluded that the Regents College examiners score the clinical examination with a high degree of consistency and accuracy, that there is no relationship between individual accuracy scores and the selected examiner characteristics studied, and that the videotaped simulation is a useful tool in evaluating the examiners' ability to score the examination.
