Its function is to assess and report on the quality and safety of services provide by the NHS and independent sector. This is the fifth and final annual 'State of Healthcare Report', as the Commission is to be replaced by the Care Quality Commission on 1 April 2009. The report is mainly about the findings for the financial year 2007-08, but it also reflects on the progress that has been made in healthcare over the past five years. The report presents six pictures of the state of healthcare: for mothers; for children and young people; for people with mental health needs; for people with a learning disability; for people needing urgent and hospital care; and of the support offered to enhance people's health and wellbeing in the community. The Commission highlights improvements, with better use and management of resources in the NHS, falling death rates from cancer and heart disease, progress in public health matters such as smoking, a greater attention to safety. Concerns remain, however: a variable picture of quality in maternity services, mental health services and urgent care; commissioning of healthcare; meeting the needs of the more vulnerable in society; children's services; care for older people. Further improvement is needed to ensure that patients really are at the centre of care, are able to make meaningful choices, be fully involved in decisions and have the information they need.

Book excerpt: The Department of Health (the 'Department') and the NHS achieved a surplus of £515 million in 2006-07, representing 0.6 per cent of total available resources. This followed two years of rising deficits, and the Department, working with the NHS, has done well in restoring overall financial balance. While the national picture is one of financial surplus there remain variations in financial performance. The surplus is concentrated in Strategic Health Authorities, whilst overall Primary Care Trusts and NHS Trusts remain in deficit Of the 372 NHS organisations, 82 recorded a deficit of £917 million, with 80 per cent of this being reported by just 10 per cent of NHS organisations. There are also regional variations, with the East of England Strategic Health Authority area having a deficit of £153 million and the North West achieving a £189 million surplus. Financial recovery is therefore inconsistent and more needs to be done so that all parts of the NHS achieve financial balance. The Committee concludes that the return to financial balance is the result of the Department's tighter performance management of NHS finances in the way funding flowed through the NHS together with a programme of support for local organisations with particular financial difficulties. In the short term, this largely centralist approach was appropriate. For the future if the NHS is to remain in financial balance more health organisations locally need to improve their financial management. Failure to keep a tight grip on financial performance will undermine health care for patients.

Holding Government to Account draws on several unique sources of evidence, including interviews with senior officials from the National Audit Office and the civil service, as well as senior parliamentarians with experience of the NAO’s relationships with government and legislature. These interviews are supplemented by an analysis of previously unpublished manuscript material in the National Archives, examination of NAO reports and parliamentary and other reports focused on accountability. Book excerpt: Holding managers to account for the resources they have been allocated is key to improving financial management at the Department for Environment, Food and Rural Affairs. This need has been recognised by the Department and reflected in a programme to improve its financial management, but the Department's Management Board recognises that establishing a culture of tighter control over its expenditure will take time. This initiative must remain a top priority and managers throughout the organisation will need to produce more reliable estimates of costs to justify their bids for resources and track the cost effectiveness of work done. The budgets agreed by the Management Board at the outset of 2006-07 and 2007-08 exceeded the funds available. In early 2006-07, increased spending to remedy difficulties with the Single Payment Scheme led to a risk of overspending in that year and the Department instigated a review which identified savings of £170 million against its original budget of £3,854 million. During the early part of 2007-08 further commitments above the agreed budget allocations meant the Department was at risk of exceeding its spending limit by £140 million. In July 2007, the Management Board identified savings which partially balanced the budget and continues to work towards a balanced budget for the year end. Effective monitoring by the Management Board and greater integration between the systems for monitoring performance delivery and financial expenditure would help better management of budgets. The NAO recommends that the Management Board set budgets from 2008-09 onwards that balance with the 2007 Comprehensive Spending Review and develops benchmarks to test the rigour of proposed budgets and to provide confirmation that these resource bids accord with the Department's strategic objectives.

Financial rsource management is relevant to every aspect of a Government department's business. By 2010-11, central government spending is forecast to grow to £678 billion, which represents £11,000 for every person in the UK. The NAO has set out a number of findings and recommendations, including: that the lack of financial skills and awareness amongst non-finance staff remains a significant barrier to improving the management of financial resources across government; that some departments lack a qualified Finance Director at Board level; that senior managers in many departments are not provided with incentives to promote sound management of financial resources; that Departments could do more to improve their forecasting capabilities; that some Departments are not sufficiently well placed to integrate financial and operational performance information; that many Departments do not always ensure the full assessment of the financial implications of policy proposals.

Thus bioethics never had to develop a normative framework sensitive to situations of disease transmission. The Patient as Victim and Vector explores how traditional and new issues in clinical medicine, research, public health, and health policy might look different in infectious disease were treated as central. The authors argue that both practice and policy must recognize that a patient with a communicable infectious disease is not only a victim of that disease, but also a potential vector- someone who may transmit an illness that will sicken or kill others. Bioethics has failed to see one part of this duality, they document, and public health the other: that the patient is both victim and vector at one and the same time. The Patient as Victim and Vector is jointly written by four authors at the University of Utah with expertise in bioethics, health law, and both clinical practice and public health policy concerning infectious disease. Part I shows how the patient-centered ethic that was developed by bioethics- especially the concept of autonomy- needs to change in the context of public health, and Part II develops a normative theory for doing so. Part III examines traditional and new issues involving infectious disease: the ethics of quarantine and isolation, research, disease screening, rapid testing, antibiotic use, and immunization, in contexts like multi-drug-resistant tuberculosis, syphilis, hepatitis, HIV/AIDS, and HPV. Part IV, beginning with a controversial thought experiment, considers constraint in the control of infectious disease, include pandemics, and Part V 'thinks big' about the global scope of infectious disease and efforts to prevent, treat, or eradicate it. This volume should have a major impact in the fields of bioethics and public health ethics. It will also interest philosophers, lawyers, health law experts, physicians, and policy makers, as well as those concerned with global health.
