Différentes approches coexistent actuellement : des méthodes d'apexification au Mineral Trioxyde Aggrgate (MTA) ou à l'hydroxyde de calcium (Ca(OH, des méthodes d'apexogénèse, ou dernièrement la revascularisation. Nous avons présenté le protocole de revascularisation pulpaire en réalisant un cas clinique dans le servive d'odontologie et de santé buccale du Groupe hospitalier Pellegrin du CHU de Bordeaux. Nous avons discuté des alternatives thérapeutiques pour ce cas, en comparant la revascularisation avec les méthodes d'apexification, en exposant les différentes désinfections canalaires possibles, et en présentant le protocole de revascularisation faisant actuellement référence. Une étude de la littérature a été faite incluant que les cases reports. On a pu remarquer qu'il existe une évolution des thérapeutiques en fonction du temps. Nous avons approfondi nos recherches sur la revascularisation, pour caractériser le tissu qui se régénère dans l'espace pulpaire laissé vacant. Des études ont prouvé qu'il ne s'agissait pas d'une régénération de tissu pulpaire. En effet un tissu ostéoide/ cémentoide dépourvu d'ostéoblaste serait la cause de l'épaississement de la racine.

Le traitement traditionnel de la dent immature nécrosée, l'apexification, tout comme l'alternative consistant à mettre en place un bouchon apical avec un ciment MTA (Mineral Trioxide Aggregate), laisse une racine avec des parois dentinaires pouvent être fines et fragiles selon son degré d'évolution lors de la nécrose. Le traitement idéal serait celui permettant le prolongement du développement de la racine et donc de l'épaississement des parois dentinaires. Cela signifie un traitement permettant une revascularisation de la pulpe nécrosée. Le but de ce travail a été de faire une revue de littérature afin d'évaluer si un protocole optimal de revascularisation des dents immatures nécrosées pouvait être mis en évidence et si des études cliniques prouvaient l'efficacité de ce dernier. Actuellement, les différents articles publiés permettent de faire ressortir un protocole qui semble admis par la plupart des auteurs. Mais la littérature ne permet pas pour l'instant d'affirmer que la revascularisation est le nouveau traitement de référence dans la prise en charge des dents immatures nécrosées. Il s'agit toutefois d'une technique prometteuse qui peut être tentée en première intention avant de revenir, si elle échoue, à un traitement plus conventionnel.

Une nouvelle thérapeutique appelée « revascularisation » a pour objectif principal d'obtenir un allongement et un épaississement des parois radiculaires ainsi qu'une fermeture apicale et, de ce fait, de renforcer la dent immature traumatisée et nécrosée. Les rapports de cas traités par cette thérapeutique novatrice et retrouvés dans la littérature internationale annoncent de tels résultats radiographiques, cependant leur valeur scientifique est faible, les données parfois subjectives et les protocoles très hétérogènes. Inversement, certains auteurs décrivent un amendement des symptômes cliniques, mais une évolution radiographique plus proche d'une réparation que d'une régénération. L'objectif de ce travail a été, dans un premier temps, de faire le point sur les résultats obtenus dans l'Unité d'Odontologie Pédiatrique du CHU de Rennes. D'un point de vue clinique, toutes les dents traitées sont asymptomatiques. En revanche, l'évolution visible à la radiographie est assez variable. Certaines dents voient une vraie reprise de leur développement radiculaire, d'autres présentent une barrière calcifiée intra-radiculaire, d'autres encore semblent rester inchangées. Dans un deuxième temps, le point a été fait sur la préparation d'un PHRC (Programme Hospitalier de Recherche Clinique) initié par le CHR de Brest en collaboration avec les CHU de Rennes et Nantes. Ce PHRC permettra de comparer la thérapeutique de revascularisation à la thérapeutique de référence (bouchon apical au MTA), de façon randomisée, sur un large spectre de population. Ses résultats devraient aider à tirer des conclusions sur le bénéfice de cette nouvelle thérapeutique, et permettre, ou non, sa généralisation.
