It is a highly contagious viral disease, and may have rapid and unanticipated national and international spread. Although not a very lethal disease for adult animals, it can cause crippling socio-economic consequences, through high production and trade losses. This manual provides information on the nature of FMD and the principles and strategic options for its prevention, control and elimination. Guidelines are provided for individual countries threatened by FMD to formulate their overall national policy on control and eradication of a possible incursion of the disease.

Book excerpt: The 2001 outbreak of Foot and Mouth was set out in a report published in 2002, providing an analysis of what happened with recommendations (HCP 888, session 2001-02, ISBN 9780102976243). Another FMD outbreak occurred in the Summer of 2007 and the Government again asked Dr Iain Anderson to conduct a review and find out if the lessons of 2001 had been learned and whether new recommendations are needed. This report is built around the findings of 2001, with a particular look at the Pirbright facility, the source of the virus incriminated in this recent outbreak. Dr Anderson's states that he found much to applaud in the handling of the 2007 outbreak along with some deficiencies, but states the positive outweighs the negative. The report commends the strong leadership of the Prime Minister, as well as the contingency planning and better integration of scientific advice and the capabilities at the centre of disease control strategies. External communication was also greatly improved. Less encouraging were the inadequate infrastructure and governance of the Pirbright facility where the Institute for Animal Health is located and the poor regulatory regime in place to licence work with dangerous animal pathogens. Defra's information systems too were found wanting. The report also recommends that the Institute of Animal Health should become the new National Institute of Infectious Diseases, supported by multiple sources of funding from government and elsewhere. Also recommended is the creation of an Independent Advisory Committee on Animal and Emerging Infectious Diseases, combining a high level group of experts along with other stakeholders with a mission to provide independent advice to government and link closely with the Government's and departmental Chief Scientific Advisors and include the Chief Veterinary Office and Chief Medical Officer amongst its members. The publication is divided into five sections: Section 1: Foreword; Section 2: Summary and recommendations; Section 3: Lessons learned, the lessons are - Lesson 1: Maintain vigilance; Lesson 2: Be prepared; Lesson 3: React with speed and certainty; Lesson 4: Explain policies, plans and practices; Lesson 5: Respect local knowledge; Lesson 6: Apply risk assessment and cost benefit analysis; Lesson 7: Use data and information management systems; Lesson 8: Have a legislative framework; Lesson 9: Base policy decisions on the best available science; Section 4: Pirbright; Section 5: Appendices.

Book excerpt: The 2001 outbreak of Foot and Mouth Disease was one of the largest and mostly costly animal disease outbreaks ever recorded, resulting in the slaughter of about six million animals with a net cost of about £2.7 billion. The European Commission subsequently disallowed 60 per cent of the UK's claim for reimbursement of compensation costs of the outbreak (judging that there had been poor control over compensation arrangements) and 80 per cent of expenditure on cleasing and disinfection of farms. Following on from a NAO report (HC 184, session 2004-05; ISBN 0102932123) published in February 2005, the Committee's report finds that good progress has been made by Defra to implement most of the reccommendations made in a previous Committee report (HCP 487, session 2002-03, ISBN 0215008693) published in March 2003. However, further action is required in two key areas, in relation to controlling the cost of a future outbreak; and contingency planning for managing future disease outbreaks.

It makes the point that given the spread of the disease before its detection, the impact of the outbreak was bound to be severe, even if everything had been handled perfectly. However there were problems and the initial response was neither fast enough nor effectively co-ordinated. MAFF took the lead but came under severe resource pressure, not only in the number of vets but also in the managerial and logistical skills that had to be later provided by the army. Because disease control policy had not been fully debated before the outbreak, arguments took place whilst the disease was raging and changes were made at short notice and were poorly communicated. In addition the full impact on tourism and the rural economy was not clearly recognised. However in Scotland, with a different management structure and closer relationships between central government, local government and the farming industry, the outbreak was better managed. As well as analysing the conduct of the Government during the epidemic, the report contains a series of recommendations to help ensure that the chances of exotic animal diseases entering the country are reduced, the farming industry is less vulnerable to outbreaks and, that if such outbreaks do occur, the impact is minimised.

Book excerpt: In March 2003 the House of Commons' Committee of Public Accounts reported on the 2001 outbreak of Foot and Mouth Disease (5th report, HCP 487, session 2002-03, ISBN 0215008693). The Committee made a number of recommendations to improve the management of future livestock epidemics by the Department for Environment, Food and Rural Affairs (DEFRA). This report examines the Government's response to the Committee in May 2003 (Cm. 5801, May 2003, ISBN 0101580126) and the progress made since then. The NAO finds that DEFRA has implemented most of the actions promised to the Committee. The main findings on the Department's preparations for a future outbreak are: (1) The Department has taken action, through improved animal health policies, to reduce the risk of an outbreak on the scale of 2001 - although further outbreaks can never be ruled out; (2) Preparedness for another outbreak is much improved in terms of contingency planning, staff training, the availability of vaccination as an adjunct to culling, improved dialogue with stakeholders and standing arrangements with contractors to make resources available to fight any future outbreak. In addition, the Department's disease control strategy is now better documented, and further research into disease control strategies is underway; (3) Some arrangements to control the cost of a future outbreak have been improved but new compensation arrangements await legislation, and discussions continue on establishing a levy scheme to share the cost of future outbreaks with the farming industry.

Book excerpt: The foot and mouth disease outbreak in Britain during 2001 resulted in over six million animals being slaughtered for disease control purposes and welfare reasons, at an estimated cost to the public sector of £3 billion and to the private sector of over £5 billion, with tourism and supporting industries worst affected. Following on from an NAO report (HC 939, session 2001-02; ISBN 0102916454), the Committee's report concludes that the Government failed to learn the lessons from the previous 1967-68 outbreak, such as the need to call in the armed forces earlier to help eradication efforts. Other necessary policy response actions, such as the immediate imposition of a national movement ban, have been learned with the benefit of hindsight. Amongst other key Committee findings, the Government's contingency planning for such an outbreak was inadequate and lacked an effective risk analysis strategy incorporating a range of assumptions about the nature, size and spread of the outbreak, including a worst case scenario. Plans were directed solely at the agricultural industry without a greater recognition of the difficulties of other industries, as well as wider rural and national concerns. In the future, there needs to be a clear policy on whether and when a vaccination scheme should be used, and this should be made known to all relevant parties. Future compensation systems need to give firmer control over the amounts paid to farmers for slaughtered animals. Measures to prevent illegal meat imports and controls on the imported livestock trade are important to avoid future outbreaks, and UK regulations need to be at least the equal of those adopted elsewhere in the developed world, including Australia, New Zealand and the US.
