On July 13, 2000, Congress enacted Public Law No. 106-246, "Making Appropriations for Military Construction, Family Housing, and Base Realignment and Closure for the Department of Defense for Fiscal Year Ending September 30, 2001, and for Other Purposes." Public Law No. 106-246 appropriated an additional $1.3 billion to cover critical shortfalls in funding managed care support contracts. In House Conference Report No. 106-710, which accompanied Public Law No. 106-246, conferees directed the Inspector General, DoD, in coordination with the General Accounting Office, to examine the fund control practices of the TRICARE (the military health care system) program. In addition, DoD was directed to report to the congressional Defense committees within 60 days after enactment on whether DoD violated fiscal law or departmental regulations related to the shortfalls. Public Law No. 106-246, section 105, specifically appropriated $615.6 million of the $1.3 billion for the Defense Health Program with the following stipulations specified in section 106.

Book excerpt: To help contain health care costs, the Department of Defense (DOD) in 1993 initiated TRICARE, its nationwide managed health care program for military personnel. By mid-1998, DOD expects to have implemented seven TRICARE managed care support contracts at an estimated cost of $17 billion. Since the award of the first TRICARE contract in 1994, 357 change orders have been made to the five TRICARE contracts now in place. Cumulatively, these change orders have increased tasks and overall costs. DOD has settled 134 of the orders at a cost of about $336,000. DOD estimates costs for the 223 orders that are yet to be settled at $38 million. However, DOD's initial cost estimates differ markedly from contractor estimates. GAO found that although there have been many change orders, DOD has not adequately managed the process. For example, rather than separately budgeting for the costs of individual change orders, DOD has used funds budgeted for other Defense Health Program activities to pay for them--an approach that could potentially create a need for supplemental funding. In addition, DOD's initial cost estimates for new orders have not been sound. As a result, DOD has not developed a reliable estimate of the total federal liability for the contract changes.

Senate Report 112-173, which accompanied a version of the National Defense Authorization Act for Fiscal Year 2013, cited concerns with the growth of DOD's health care costs and identified private sector health care contracts as an area for potential savings and efficiencies. The Senate report mandated that GAO review DOD's process for acquiring TRICARE's MCSCs. This report examines: (1) TMA's acquisition process to award TRICARE's third generation MCSCs; (2) the extent to which issues were raised in the bid protests involving these MCSCs, including identifying any common themes; and (3) lessons learned from the acquisition process to award these MCSCs and how these lessons may be used in future acquisitions.

Through TRICARE, its new managed health care program, DOD is trying to improve access to care among its 8.3 million beneficiaries while containing costs. How well DOD implements and operates TRICARE may define and shape military medicine for years to come. Because of TRICARE's complexity, scale, and impact on beneficiaries, GAO reviewed the program, focusing on (1) whether DOD's experiences with early implementation yielded the expected results, (2) how early outcomes may affect costs, and (3) whether DOD has defined and is capturing data needed to manage and assess TRICARE's performance. GAO concludes that despite initial confusion among beneficiaries arising from marketing and education problems, as well as problems with the compatibility of computer systems, early implementation of TRICARE is progressing consistent with congressional and DOD goals. However, the success of DOD's efforts to implement resource-sharing agreements and utilization management is critical to containing health care costs. DOD also needs to gather enrollment and performance data so that it and Congress can assess TRICARE's success in the future.

Almost half of DoD's medical research funding is administered by the Congressionally Directed Medical Research Programs (CDMRP). The mission of CDMRP is to foster innovative approaches to medical research in response to the needs of its stakeholdersâ€"the U.S. military, their families, the American public, and Congress. CDMRP funds medical research to be performed by other government and nongovernmental organizations, but it does not conduct research itself. The major focus of CDMRP funded research is the improved prevention, diagnosis, and treatment of diseases, injuries, or conditions that affect service members and their families, and the general public. The hallmarks of CDMRP include reviewing applications for research funding using a two-tiered review process, and involving consumers throughout the process. Evaluation of the Congressionally Directed Medical Research Programs Review Process evaluates the CDMRP two-tiered peer review process, its coordination of research priorities with NIH and the Department of Veterans Affairs, and provides recommendations on how the process for reviewing and selecting studies can be improved.

TRICARE is a complicated health care system with several different parts. One key component is Prime, the managed care portion of the Defense Health Plan (DHP). One must enroll in Prime in order to receive care under it; however, other options for receiving care do not require enrollment. This study responds to tasking from the Under Secretary of Defense (USD) for Personnel and Readiness concerning the feasibility of an enrollment system for the DHR Under Prime, enrollment is a requirement for receiving care. In a limited sense, enrollment is not only possible but currently under way. We believe, however, that the more important question and one posed under the tasking is whether universal enrollment is feasible. As we'll show, Prime pertains to a relatively important and growing part of the beneficiary population that relies on military treatment facilities (MTF)-military clinics and hospitals-for health care. The other user of the MTFs rely on space-available care. These people don't have to enroll to use military healthcare providers or facilities; they use the NTFs for care when there is sufficient capacity.
