Book excerpt: In an ideal world, the National Security Strategy (NSS) drives the National Military Strategy (NMS), the Joint Strategic Capabilities Plan, the Defense and Contingency Planning Guidance, and ultimately the Future Years Defense Program (FYDP) budgetary request to Congress. If resources are unlimited, all aspects of NMS and NSS goals are theoretically achievable. In reality, resource constraints limit the FYDP and hence achievable strategic goals. This forces the US to prioritize national interests, selecting some to pursue and some to abandon. Resource allocation (spending) is strategic. In our open democratic process, regional or special interest groups can force allocation of resources to goals not necessarily of the highest global priority. Actions by the Defense Department may facilitate action by special interest groups, producing a significant reallocation of defense resources. Ideally, the Defense Department recognizes this possibility when it makes decisions, makes allowances for the possible outcomes, and ultimately makes an informed decision on the wisdom of a particular action and its present and future impact on our NSS. Unfortunately, the Defense Department is not always sufficiently prescient in its appreciation of the consequences of its actions. This report reviews one such instance, the promise of free medical care for life, and its potential impact on future resource allocation. This case study also demonstrates the intricate interplay of political and legal systems on resource allocation and, by extension, on NMS and NSS.

Book excerpt: Rising health care costs are placing a considerable burden on Government and Department of Defense resources. At the current rate, health care costs will likely impact the overall readiness and future force structure of the Department of Defense. In efforts to reduce or contain the costs of the military health care system, this project will propose two cost savings alternatives for providing outpatient care to active duty personnel and their dependents. In addition to analyzing the current system as an alternative (status quo), the two proposed cost-sharing alternatives are a standard co-payment and a monthly allotment program. These strategies are primarily designed to reduce Department of Defense costs by influencing the attitudes of beneficiaries toward outpatient health care, restricting their access to options with higher costs, and shifting a portion of outpatient care costs through co-payments. This project provides an in-depth analysis for each alternative using a set of assumptions for treatment possibilities based on three likely scenarios, and provides quantitative and qualitative data to arrive at a recommendation. Using acquisition principles, this project provides an implementation process for the proposed alternative that should minimize disruption in the current military health care system. This project highlights the hypothesis by implementing a cost-saving strategy to active duty and dependent outpatient care; thus, the military health care system will experience a significant cost reduction over the long run.

These challenges must be considered in the contexts of the current and ongoing needs of Active Duty, National Guard and Reserve military personnel, of their families, of the aging military retiree population, and the broader backdrop of the U.S. health care economy, in which the military health care system operates. The DoD must preserve the quality of care in order to ensure positive unit and family morale during these very trying times. The MHS must improve certain practices to ensure force readiness and provision of the highest quality health care. The all-volunteer force should never have any doubt that they and their families will always receive first class care.

Book excerpt: "The cost of military health care has almost tripled over the last decade from $19 billion in 2001to $53.5 billion in 2012 and is approaching 10% of the defense budget. This unprecedented growth, and the huge cuts to the overall Department of Defense (DoD) budget, is driving strategic tradeoffs between competing defense priorities, including how much the DoD can afford to pay for health care. The DoD has repeatedly proposed higher enrollment fees in order to help offset health-care costs and modify beneficiary utilization behavior, but it has been unable to secure approval through Congress. Most experts agree fundamental reform to the Military Health System (MHS) is needed, but few offer concrete, politically viable options. This paper will analyze and evaluate the costs, benefits and policy implications associated with redefining the role of employers of working military retirees and improving cost-share parity among retirees. Specifically, I recommend employers be required to pay TRICARE the insurance premium they would have otherwise had to pay to their insurance carrier if the retiree opts out of the employer plan. Just as important, I propose DoD leave the baseline TRICARE fees alone, but charge an additional fee to working-age retirees based on a percentage of wages above retirement pay. This will require changes to Section 707 of the 2007 NDAA as it relates to employers providing payments to retirees for opting out of employer-sponsored health insurance coverage. Implementing these proposed options could provide the MHS an additional $48 billion in revenue across the Future Years Defense Program (FYDP, FY 2015-2019). In this period of fiscal austerity, I fear significant cuts to the MHS could have unintended, long-lasting, negative impacts to military readiness. The funding provided through my proposal has the potential to maintain the current benefit and provide additional time for deliberate strategic planning."--Abstract, page iv.

Best Care at Lower Cost explains that inefficiencies, an overwhelming amount of data, and other economic and quality barriers hinder progress in improving health and threaten the nation's economic stability and global competitiveness. According to this report, the knowledge and tools exist to put the health system on the right course to achieve continuous improvement and better quality care at a lower cost. The costs of the system's current inefficiency underscore the urgent need for a systemwide transformation. About 30 percent of health spending in 2009-roughly $750 billion-was wasted on unnecessary services, excessive administrative costs, fraud, and other problems. Moreover, inefficiencies cause needless suffering. By one estimate, roughly 75,000 deaths might have been averted in 2005 if every state had delivered care at the quality level of the best performing state. This report states that the way health care providers currently train, practice, and learn new information cannot keep pace with the flood of research discoveries and technological advances.
