Book excerpt: L'objectif de cette étude est d'évaluer un nouvel algorithme de reconstruction tomodensitométrique basé sur le Deep Learning (DLR) et de le comparer en termes de qualité d'images et de réduction de dose d'irradiation aux reconstructions itératives classiques (hybrid-IR). Cent quarante patients consécutifs adressés pour la réalisation d'un angioscanner thoracique pour suspicion d'embolie pulmonaire ont été inclus. Cinquante patients (50%) ont bénéficié d'un scanner avec DLR et cinquante patients d'un scanner avec hybrid-IR. La qualité d'image a été évaluée quantitativement (niveau de bruit ; rapports signal-sur-bruit, SNR et contraste-sur-bruit, CNR) et qualitativement sur une échelle à 5-points. Les paramètres d'irradiations ont été recueillis (CT dose index, CTDIvol ; et le produit dose longueur, DLP). Au total, 93 patients ont été analysés : 45 avec DLR et 48 avec hybrid-IR. Disadvantages of the conventional single-energy technique used by CT technology are that different materials cannot be distinguished and that the noise is larger. To address these problems, a novel spectral CT concept has been proposed. Spectral Dual-Energy CT (DE-CT) acquires two sets of spectral data, and Spectral Photon Counting CT (PC-CT) detects energy of x-ray photons to reveal additional material information of objects by using novel energy-sensitive, photon-counting detectors. Matériels/Méthodes.147 patients consécutifs d'un service d'urgences d'un hôpital universitaire entre 2015 et 2016 référés pour une douleur thoracique ont été évalués par TRO-CTA. Les patients étaient assignés de manière randomisée à un protocole standard dose à 120 kVp (n=50, 66.6 ± 19.0 ans, 66% homme) ou basse dose à 100 kVp (n=97, 62.8 ± 17.9 ans, 67% homme). Le bruit dans l'image, l'atténuation dans les vaisseaux, les rapports signal-à-bruit et contraste-à-bruit dans 5 vaisseaux thoraciques majeurs ont été mésurés pour une analyse objective et subjective.
