Book excerpt: One of the most demanding aspects regarding the endocrinology of aging is to determine the extent to which hormones change with age, independent of age-related pathologies, and the clinical significance of these changes. The emphasis of the present volume is on function, as it is of particular interest to many nonspecialists under what circumstances endocrine therapy may be useful in the elderly. For this purpose, leading investigators in their respective fields review the most important hormonal systems affected by aging. Furthermore, to facilitate the utility of the reviews particularly for nonspecialists, the book emphasizes the critical analysis of specific issues: The extent to which the hormones considered really do change with age, especially in humans; the physiological significance and the causes of these changes; and the prospects for treatment. Additionally, this volume also includes reviews on subjects such as cytokines, the sympathetic nervous system and leptin, which have not been traditionally considered in reviews of endocrine systems. However, while the concept of hormone replacement in the elderly is promising, as the chapters in the present volume indicate, much more research is necessary to determine under what circumstances such therapy is appropriate rather than counterproductive.

At the beginning, it was a tenuous alliance between physicians in quite different medical subspecialties-urology and psychiatry. As the alliance was forged, subspecialty rivalries and mistrust were replaced by a common clinical interest in the diagnosis and treatment of erectile problems. We quickly became aware of the high prevalence of such disorders, how poorly prepared we were to make accurate and re sponsible diagnosis and treatment plans, and how complicated an etiol ogical diagnosis could prove to be. A variety of biological and psychological influences bear on sexual function, and in many clinical contexts, diagnosis and treatment planning involves consideration of complex interactive var iables. The need for an expanded multidisciplinary team became obvious. The requisite knowledge base extended across too many subspecialty boundaries, and the necessary information was not available in a conve nient source. As we began accumulating the information base, we became aware that this information might be of value to other physicians. The authors enjoy the luxury of an academic setting in which special expertise can be readily assembled. Such resources are often unavailable to the physician in practice. In others it is, but the available conclusions the information supports block its acceptance, because they conflict with the posi tions of dominant groups in the politics of sexuality. Possibly the most obvious example is the determination of many theorists to ignore the evidence that while men rarely report being sexually assaulted, when questioned in community surveys, they make up a third of the victims, and a quarter of the perpetrators of sexual assault are women. These findings are incompatible with the feminist theory that sexual assault is not a sexual act, but normal male behavior motivated politically, to maintain men's dominance of women. Most research supporting the theory investigated women only as victims and men only as aggressors. Some feminists have dismissed the need for any research to support their beliefs on the ground that such research is "busy work" establishing what women already know. One belief considered not to require re search is that heterosexual pornography made for a male audience increases the prevalence of rape by encouraging rape-supportive attitudes of male aggression and female submission. No criticism has been directed at the soft-core pornography of "Mills and Boon" stories written for women that promote similar attitudes.
