Book excerpt: Transfusion safety is a significant concern both to the public, who may at some time require a blood transfusion, and to blood collection and transfusion professionals. However, blood transfusion, like most other medical procedures, will never be free of risk. A review of risk reduction strategies applied since the identification of AIDS as a transfusion-transmitted disease suggests that some have been implemented due to political and legal pressures rather than for scientifically based medical reasons. Without the benefit of a standard mechanism for assessing when these actions will result in measurable risk reduction, decisions about transfusion safety will continue to be driven by public opinion. A model was developed for evaluating when and if a potential laboratory test or other approaches will improve safety. the model addresses recipient risk factors such as the prevalence of the disease-causing agent in the population, the severity of the disease and the public health impact as well as operational issues including potential depletion of the donor pool, the impact of donor deferral and the financial impact. Application of the model to prior actions taken by blood providers to reduce the risk of transfusions documents its validity as a decision-making tool. Adoption of such a standardized decision-making approach would assure recipient safety while controlling the costs of providing blood for transfusion purposes.

So, it has been a long time since. After having been away from Groningen for 18 years I have been inaugurated last week as a mayor and it is a plcasure to be again in your midst. The scries of annual symposia on blood transfusion have contributed to mark the city of Groningen on the map of the international scientific world. A great number of prestigious institutes all over the world involved in the development of transfusion medicine have linked to Groningen and we are proud of that.
