L'objectif de cette thèse est d'identifier les obstacles au diagnostic précoce des démences en Médecines Générale. Cette enquête, réalisée auprès de 15 Médecins Généralistes de l'Allier, au moyen d'entretiens individuels semi-dirigés, a permis d'étudier leur attitude face à la plainte mnésique ainsi que les difficultés rencontrées dans la prise en charge du patient dément. Les difficultés s'avèrent nombreuses. Du point de vue du Médecin, la difficulté à repérer les signes précoces évocateurs de démence ainsi que le manque de temps sont les principaux écueils. Le déni du patient représente un obstacle majeur, au même titre que celui de son entourage aux stades les plus précoces de la maladie. Le diagnostic précoce pourrait être favorisé par la possibilité d'obtenir facilement une prise en charge pluridisciplinaire optimale du patient dément. Enfin, la plainte mnésique, même si elle est entendue par les Médecins Généralistes, revêt une valeur encore trop souvent sous-estimée. La majorité des médecins interrogés estime que le diagnostic précoce est important et les raisons évoquées sont variées : tantôt dans l'intérêt du patient lui-même, tantôt dans l'intérêt de l'entourage, dans l'attente de traitements plus efficaces et toujours dans l'idée de maintenir une qualité de vie satisfaisante à domicile.
