Mais loin de pouvoir considérer l'intitulé judiciaire d'agresseur sexuel comme une réalité médicale, il faut s'interroger sur les propositions nosologiques qui sont à notre disposition. Et l'on aura alors rapidement recours à une évaluation médicopsychologique multidisciplinaire, tant la seule psychiatrie montre à ce jour les limites de son champ d'investigation dans ce domaine. C'est le repérage de problématiques psychopathologiques polymorphes qui permet alors de mener une réflexion pertinente sur la question de l'indication thérapeutique. Nous avons ainsi mené une évaluation médicopsychologique de 7 sujets auteurs d'agressions sexuelles sur mineurs incarcérés à la maison d'arrêt du Mans. Dans un deuxième temps nous avons cherché, par une analyse pluridisciplinaire des données cliniquement recueillies, à préciser les organisations psychopathologiques de ces sujets. Complétant cette analyse par un aperçu de la littérature sur la question des groupes de parole, nous avons dégagé un certain nombre de critères d'indications thérapeutiques de groupe de parole, et avons alors posé une indication thérapeutique pour quatre des sept sujets évalués. Notre étude se prolonge alors par la mise en place d'un groupe de parole fermé, à médiation par un jeu de carte, le "qu'en dit-on", et se termine par l'analyse des données recueillies lors de la réalisation du groupe, dans un référentiel psychodynamique.

L'objectif de la loi est très clair : inciter aux soins dans le but de prévenir la récidive. Pour y parvenir, les facteurs de risque statiques et dynamiques devraient être considérés ce qui nécessite une formation spécialisée en criminologie. L'objectif de cette recherche est de mettre en évidence, d'une part les éléments principaux à évaluer avant toute prise en charge par thérapie comportementale et cognitive, et d'autre part à sélectionner des outils qui permettent une meilleure évaluation des auteurs de violence sexuelle sur enfants (AVSE).Méthode : un entretien semi-structuré a été développé pour obtenir une conceptualisation et une contextualisation générales des violences sexuelles sur les enfants. Cet entretien, associé à une évaluation des troubles mentaux et de personnalité, a été évalué auprès de 19 hommes condamnés par la justice française pour des crimes et délits commis à l'encontre de mineurs de moins de quinze ans. Dans une seconde étude, nous avons étudié la désirabilité sociale (DS) et les trois dimensions de la personnalité (extraversion, névrosisme et psychoticisme) dans ce même groupe d'AVSE. Dans la mesure où nous ne disposions pas de normes françaises à l'échelle de DS, nous avons étudié cette variable dans un groupe de 418 sujets issus de la population générale. Nous avons comparé les résultats des 19 AVSE avec ceux de deux groupes contrôles : des sujets violents (n=19) et non violents (n=19). Notre troisième étude nous a permis d'analyser l'activation de schémas précoces inadaptés (SPI) dans notre échantillon clinique et de comparer ces résultats à nos deux groupes contrôles.Résultats : la première étude nous a permis de définir les éléments importants à évaluer par tout professionnel du soin formé en thérapie comportementale et cognitive : les expériences d'adversité vécues pendant l'enfance, l'estime de soi, la sphère émotionnelle, l'assertivité, la sexualité normale, la vision négative passée de la conjointe et de la femme, le rapport à autrui, les théories implicites et les schémas cognitifs. L'étude de la DS en population générale nous a fourni des valeurs de référence de la version francophone de l'outil. Notre première hypothèse (les scores des AVSE à l'échelle de DS sont supérieurs à ceux des sujets non violents) est validée. Notre seconde hypothèse (les scores des AVSE à l'échelle de DS sont supérieurs à ceux de sujets violents) n'est pas validée. Concernant les SPI, notre première hypothèse (les AVSE ont des scores plus élevés à l'échelle que les sujets non violents) est validée et notre deuxième hypothèse (les AVSE obtiennent des scores plus élevés que les sujets non violents aux SPI surcontrôle émotionnel, isolement social, méfiance/abus, abandon/instabilité, assujetissement et imperfection/honte est validée partiellement.Discussion : la conceptualisation de la problématique de l'AVSE avant une prise en charge comprend les neuf éléments retenus à partir de l'analyse de contenu des entretiens. Nos trois études nous ont permis de préconiser également l'évaluation des besoins fondamentaux, des buts, des valeurs associés et les stratégies de résolution de problème. Une mesure de la qualité de vie serait un atout. Associés à ces variables, plusieurs outils d'évaluation se sont révélés pertinents : l'entretien structuré MINI International Neuropsychiatric Interview pour évaluer les troubles mentaux, le Personnality Disorder Questionnaire 4+ pour évaluer les troubles de personnalité, l'échelle abrégée de désirabilité sociale forme C et le Young Schema Questionnaire-Short form 3 pour étudier les SPI. Nous recommandons l'Eysenck Personality Questionnaire Revised pour évaluer les trois dimensions de la personnalité et l'échelle auto-rapportée Self-Report Psychopathy Scale-III pour évaluer les quatre facettes de la psychopathie.

L’évaluation et la prise en charge de ces sujets ont mis en lumière la place considérable de cette problématique dans notre société, et ce dans le but d’améliorer la qualité des soins et la prévention de la récidive. D’après la littérature contemporaine, des perturbations de l’empathie sont fréquemment rencontrées chez ces patients, et notamment envers leurs victimes. L’empathie peut être définie comme la capacité à partager et à comprendre l’émotion d’autrui, tout en se distinguant de l’autre. Elle est au cœur de la relation intersubjective, et joue un rôle prépondérant dans la création et le maintien des liens sociaux. Ce travail tentera de présenter l’empathie à travers son histoire, ses définitions actuelles, son approche cognitive, ses outils de mesure et ses liens avec l’alexithymie (incapacité à identifier et à décrire ses propres émotions). Puis, il sera question d’aborder l’aspect clinique et thérapeutique concernant les auteurs d’agression sexuelle à caractère pédophile. Point central dans la mise en place des programmes de traitement basés sur des approches cognitivo-comportementales, l’empathie chez les auteurs d’agression sexuelle à caractère pédophile sera ensuite évaluée, au travers d’une étude effectuée auprès d’une population carcérale de 63 sujets, à l’aide d’un auto-questionnaire : la “Child Molester Empathy Measure” (CMEM). Ayant déjà mis en évidence une prévalence élevée d’alexithymie chez les auteurs d’agression sexuelle à caractère pédophile, mais sans différence significative avec des sujets-contrôle, nous en profiterons pour explorer les relations entre empathie et alexithymie

Under the rubric of violence against women the two overlapping issues have received increasing attention. Much of the response to these forms of violence has focussed either on advocacy, providing adequate health and legal services and ensuring safety of victims once violence is disclosed, or on screening for violence in the context of health services. When the word "prevention" is used, it is usually in reference to secondary and tertiary prevention - helping women get out of violent situations and preventing further violence. Little attention has been given to primary prevention - addressing the root causes with the goal of reducing the number of new cases. Recent hallmark publications have called for increased investment in primary prevention. As part of the Global Campaign for Violence Prevention, WHO is giving increased attention to the primary prevention of intimate partner violence and sexual violence. Preventing intimate partner and sexual violence against women: taking action and generating evidence is an important new tool for violence prevention researchers, practitioners and advocates provides a planning framework for developing policies and programmes for the prevention of intimate partner and sexual violence. This guide outlines the nature, magnitude, risks and consequences of intimate partner and sexual violence. It outlines strategies to prevent these forms of violence against women and describes how these can be tailored to the needs, capacities and resources of particular settings. It also emphasizes the importance of integrating scientific evaluation into all prevention activities in order to expand current knowledge of what works. The manual describes interventions of known effectiveness, those supported by emerging evidence, and those that could potentially be effective, but have yet to be sufficiently evaluated for their impact. For instance, school-based programmes to prevent violence occurring in "dating relationships" have been shown to be effective in randomized trials in the USA and Canada. Evidence is emerging for the effectiveness of empowerment approaches which use microfinance combined with gender-equality training or are based on communications and relationship skills training such as the Stepping Stones training package, which has been widely used in low- and middle-income countries. Strategies that aim to prevent intimate partner and sexual violence through reducing the harmful use of alcohol also show promise. A six-step framework for planning, implementing and evaluating such prevention programmes and policies is presented. This document is primarily aimed at policy-makers, programme planners and donors from public health and related sectors who seek to advance the prevention of intimate partner and sexual violence against women. In addition to the principal audience, other interested parties will include those working in other government sectors such as education, child welfare, social care, criminal justice and departments of women or gender equality; advocates from civil society organizations; local authorities; environmental and urban planners; and researchers."
