Book Health Care Evaluation Using Computer Simulation

Download or read book Health Care Evaluation Using Computer Simulation written by Boris Sobolev and published by Springer Science & Business Media. This book was released on 2012-07-12 with total page 419 pages. Available in PDF, EPUB and Kindle. Book excerpt: The purpose of this book is to place computer simulation studies within the paradigm of intervention research that is concerned with comparing the outcomes of health care delivered under different policies. This book presents computer simulation as a tool for testing various policy alternatives that have been developed by decision-makers within health care systems. This approach differs from the use of computer simulation in operations research, where simulation helps determine the configurations of a system that will allow it to function optimally. Although simulation of health care processes is not new, few health care systems have used simulations as a basis for re-engineering the delivery of health services. There is growing appreciation that the complexity of health care processes exceeds the capacity of individual disciplines–health services research, health economics, or operations research–to guide health care reform. In this book, the authors focus on bringing the methodological rigor of evaluative research to the design and analysis of such simulation studies. The book is intended as a reference for health services researchers. It offers a comprehensive description of the methodology of conducting simulation studies in evaluation of service alternatives in surgical care using discrete-event models, including the steps for identifying the clinical and managerial activities of the perioperative process, determining the model requirements, implementing simulation models, designing simulation experiments and analyzing the experimental data, and interpreting and reporting results. The book also offers examples of specific aspects of conducting simulation experiments: how to determine the number of runs needed to estimate the effect of implementing a health care policy; how to allocate the number of runs to study groups in simulation experiments aiming to evaluate policy or management alternatives; and how to use statistical analysis to estimate, interpret, and report effect sizes.

Book Integrated Simulation Model for Patient Flow Between Operating Rooms and Progressive Care Units Using Custom Objects

Download or read book Integrated Simulation Model for Patient Flow Between Operating Rooms and Progressive Care Units Using Custom Objects written by Ryan L. Miller and published by . This book was released on 2020 with total page 109 pages. Available in PDF, EPUB and Kindle. Book excerpt: Process improvements in hospitals usually focus on a single department (eg. emergency department, operating theater, specialty clinic, etc). However, actions taken in one department inevitably affect the performance of other departments. Therefore, higher efficiency improvements can be obtained by considering the patient care process as one synergetic activity involving several departments and various sets of resources. In this research we propose an integrated approach for modeling the patient lifecycle for multiple departments. First we describe a patient flow from his/her entry into the hospital through a progressive care unit until the patient has fully recovered. We use process mapping methods to address value added activities and other necessary activities in the patient lifecycle. Then, a simulation model is developed in Simio using customized objects created in previous works. Those customized objects carry their own logic and behavior. For example, the Bed object includes logic for a patient recovering while using several hospital resources (nurses, therapist) in his/her hospital stay. Those objects were used to build several configurations of an integrated model with multiple departments. Data about patient arrival patterns, their health acuity, and procedure needs were obtained from a real hospital in order to test our approach. The procedures duration data (which were different for different levels of patient acuity and for different surgical and other procedures) were used to obtain service distribution using statistical analysis methods. Modular simulation objects and data distributions from real hospitals allowed us to build an integrated simulation model with several configurations of the process flow. Simulation experiments were performed on these models and performance recorded. The recommendation for implementations in the hospitals is also reported.

Book Healthcare Service Management

Download or read book Healthcare Service Management written by Li Tao and published by Springer. This book was released on 2019-05-08 with total page 168 pages. Available in PDF, EPUB and Kindle. Book excerpt: Healthcare service systems are of profound importance in promoting the public health and wellness of people. This book introduces a data-driven complex systems modeling approach (D2CSM) to systematically understand and improve the essence of healthcare service systems. In particular, this data-driven approach provides new perspectives on health service performance by unveiling the causes for service disparity, such as spatio-temporal variations in wait times across different hospitals. The approach integrates four methods -- Structural Equation Modeling (SEM)-based analysis; integrated projection; service management strategy design and evaluation; and behavior-based autonomy-oriented modeling -- to address respective challenges encountered in performing data analytics and modeling studies on healthcare services. The thrust and uniqueness of this approach lies in the following aspects: Ability to explore underlying complex relationships between observed or latent impact factors and service performance. Ability to predict the changes and demonstrate the corresponding dynamics of service utilization and service performance. Ability to strategically manage service resources with the adaptation of unpredictable patient arrivals. Ability to figure out the working mechanisms that account for certain spatio-temporal patterns of service utilization and performance. To show the practical effectiveness of the proposed systematic approach, this book provides a series of pilot studies within the context of cardiac care in Ontario, Canada. The exemplified studies have unveiled some novel findings, e.g., (1) service accessibility and education may relieve the pressure of population size on service utilization; (2) functionally coupled units may have a certain cross-unit wait-time relationship potentially because of a delay cascade phenomena; (3) strategically allocating time blocks in operating rooms (ORs) based on a feedback mechanism may benefit OR utilization; (4) patients’ and hospitals’ autonomous behavior, and their interactions via wait times may bear the responsible for the emergence of spatio-temporal patterns observed in the real-world cardiac care system. Furthermore, this book presents an intelligent healthcare decision support (iHDS) system, an integrated architecture for implementing the data-driven complex systems modeling approach to developing, analyzing, investigating, supporting and advising healthcare related decisions. In summary, this book provides a data-driven systematic approach for addressing practical decision-support problems confronted in healthcare service management. This approach will provide policy makers, researchers, and practitioners with a practically useful way for examining service utilization and service performance in various ``what-if" scenarios, inspiring the design of effectiveness resource-allocation strategies, and deepening the understanding of the nature of complex healthcare service systems.

Book Discrete Event Simulation of Operating Rooms Using Data driven Modeling

Download or read book Discrete Event Simulation of Operating Rooms Using Data driven Modeling written by Mandvi Malik and published by . This book was released on 2018 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: Simulation has been extensively used in various studies to optimize the healthcare facility today. However, the simulation models built for the healthcare industry in the past are mostly made using a model-driven approach. The simulation models made using model-driven approach are not easy to use and modify by someone who is not familiar with simulation tools. To address this shortcoming, the focus of the present research is to create a data-driven simulation model of operating rooms. The focus is to create a simulation model that can be applied to create a simulation model of any operating room unit. For this purpose, the simulation model is built with the combination of customized objects and data-driven add-ins.
