Book excerpt: État des lieux : À la Réunion, il existe une surmortalité maternelle deux fois plus élevée qu'en métropole sur la période 2006-2010. Il en est de même pour la mortalité infantile. C'est pourquoi l'amélioration des indicateurs en périnatalité constitue une priorité du nouveau plan régional 2012-2016 et du réseau périnatal REPERE qui a créé le projet SUPREME afin de favoriser le développement d'outil d'aide à la pratique pour les praticiens libéraux. Objectifs : Créer un outil d'aide à la pratique tel qu'un questionnaire auto-administré de recueil de données médicales chez la femme enceinte, puis recueillir l'avis d'un échantillon de médecins généralistes sur son utilisation en pratique afin de mieux repérer les grossesses à haut risque. Méthodes : Le questionnaire a été élaboré sur les bases des recommandations du CNGOF et de la HAS. Il a ensuite été soumis à un pré-test auprès d'un échantillon de patientes et d'un focus groupe, puis le prototype obtenu a subi un test en pratique courante auprès d'un échantillon de praticiens et leurs patientes durant 10 semaines. Enfin, nous avons recueilli l'opinion de ces derniers suite à son utilisation grâce à une enquête d'opinion. Résultats : 34 médecins généralistes libéraux ont testé le questionnaire auto-administré et ont participé à l'étude. Parmi eux, il y a autant d'hommes que de femmes (17) et autant appartenant (16) ou non au réseau REPERE. 70.58% (24) sont installés en cabinet de groupe et 67.65% (23) depuis plus de 15 ans. 55.88% (19) exercent en milieu rural et pour 85.30% (29) d'entre eux, dans l'ouest et le sud de l'île. 58.82% (20) sont âgés de plus de 50 ans. 85.29% (19) suivent moins de 20 grossesses par an et 67.66% (22) les suivent jusqu'au troisième trimestre. Le questionnaire a été jugé comme étant facile ou très facile à utiliser pour 91.18% (31) des praticiens et utile dans leur pratique courante pour 76.48% (27) d'entre eux. Ce dernier a également été jugé à 79.42% (27) comme étant un outil permettant de rendre plus exhaustif le recueil des informations médicales, ainsi que pouvant permettre une plus grande participation de leurs patientes lors de la consultation pour 61.77% (21) d'entre eux. Sa longueur a été jugée adaptée par 76.48% (26) des médecins. 85.24% (30) des praticiens sont d'accord pour utiliser cet outil dans leur pratique quotidienne du suivi de la grossesse, mais 58.82% (20) ne l'utiliseraient pas chez toutes leurs patientes. Du côté des 79 patientes, 89.87% (71) d'entre elles ont jugé le questionnaire facile à remplir et 67.09% (53) ont trouvé qu'il les a amenées à réfléchir sur leur grossesse. Conclusion : Notre auto-questionnaire administré chez la femme enceinte a été jugé par les praticiens libéraux de notre étude, comme étant un outil utile au besoin d'améliorer le suivi de la grossesse sur l'île de la Réunion. Des travaux supplémentaires tels qu'un test de faisabilité pourront être proposés afin de juger de son utilité au dépistage des risques de la grossesse.

Yet in 1992-93, some five hundred people died from cholera in the Orinoco Delta of eastern Venezuela. In some communities, a third of the adults died in a single night, as anthropologist Charles Briggs and Clara Mantini-Briggs, a Venezuelan public health physician, reveal in their frontline report. Why, they ask in this moving and thought-provoking account, did so many die near the end of the twentieth century from a bacterial infection associated with the premodern past? It was evident that the number of deaths resulted not only from inadequacies in medical services but also from the failure of public health officials to inform residents that cholera was likely to arrive. Less evident were the ways that scientists, officials, and politicians connected representations of infectious diseases with images of social inequality. In Venezuela, cholera was racialized as officials used anthropological notions of "culture" in deflecting blame away from their institutions and onto the victims themselves. The disease, the space of the Orinoco Delta, and the "indigenous ethnic group" who suffered cholera all came to seem somehow synonymous. One of the major threats to people's health worldwide is this deadly cycle of passing the blame. Carefully documenting how stigma, stories, and statistics circulate across borders, this first-rate ethnography demonstrates that the process undermines all the efforts of physicians and public health officials and at the same time contributes catastrophically to epidemics not only of cholera but also of tuberculosis, malaria, AIDS, and other killers. The authors have harnessed their own outrage over what took place during the epidemic and its aftermath in order to make clear the political and human stakes involved in the circulation of narratives, resources, and germs.
