Book excerpt: L'insomnie augmente avec l'âge et est responsable d'une forte consommation d'hypnotiques, le plus souvent prolongée, en particulier chez les sujets âgés, malgré leurs nombreux effets secondaires et les différentes recommandations. Cette étude cherche à mettre en évidence les facteurs déterminants de la prescription des hypnotiques dans l'insomnie du sujet âgé en médecine générale. Il s'agit d'une étude qualitative par entretiens semi-dirigés auprès de 11 médecins généralistes, exerçant dans la région Languedoc- Roussillon entre février et avril 2016. Cette étude s'intéresse aux 3 temps de la prescription: l'initiation, le renouvellement et l'arrêt. Elle met en évidence des déterminants de la prescription liés au médecin (l'insuffisance de formation, la relation médecin-malade), liés au patient (l'état clinique, le ressenti), liés aux hypnotiques (la durée, les effets), liés aux troubles du sommeil (la durée, le retentissement), liés à l'environnement (EHPAD, entourage) et liés à la condition d'exercice du médecin (le manque de temps, l'exercice isolé de la médecine). Ce travail montre les nombreuses difficultés des médecins généralistes à la prise en charge de l'insomnie. Plusieurs pistes sont à envisager dont le renforcement de la formation des médecins généralistes, des campagnes de sensibilisation du personnel paramédical et du public ou encore faciliter l'accès aux spécia listes et aux thérapies cognitivo-comportementales

Book excerpt: Contexte : les sujets âgés sont une population particulièrement à risque de développer une insomnie en raison du développement d'une fragilité. Les troubles du sommeil constituent un syndrome gériatrique auquel le praticien doit être sensibilisé. L'optimisation des connaissances et des pratiques médicales est essentielle afin d'envisager des mesures préventives et thérapeutiques adéquates. Selon les dernières recommandations européennes de l'insomnie, le traitement le plus efficace de l'insomnie est la thérapie cognitive et comportementale. Malgré cela cette thérapeutique est cependant encore peu sollicitée et cela au détriment d'une surutilisation de médicaments hypnotiques malgré un ratio qualité de sommeil/effets indésirables défavorable. L'objectif de cette étude est d'observer l'impact d'une formation des médecins à la prescription des hypnotiques sur les niveaux de prescription en milieu hospitalier chez les sujets gériatriques dans la prise en charge de l'insomnie.Méthodes : l'étude était prospective, monocentrique, sur une période de quatre mois, avec recueil factuel des données portant sur les pratiques des médecins via le dossier médical patient informatisé. La population étudiée était constituée des malades gériatriques âgés de plus de 70 ans, hospitalisés dans plusieurs services hospitaliers du Centre hospitalier universitaire (CHU) de Nice. Étaient exclus de l'étude, les patients hospitalisés dont les médecins étaient absents à la formation. Création d'un recueil de donnée des caractéristiques de la population et des prescriptions d'hypnotiques une semaine avant la formation ; réalisation une semaine après d'une formation sur la prise en charge de l'insomnie et bon usage des hypnotiques chez le sujet âgé à un total de 34 médecins ; recueil des données à 15 jours, un mois et trois mois après formation. Le critère de jugement principal était l'évolution du taux de prescription des hypnotiques après formation à la bonne prescription des hypnotiques. Comparaison des pourcentages de prescription une semaine avant formation, à 15 jours, un mois et trois mois après formation. Résultats : 840 patients gériatriques ont été inclus (232 avant formation et 608 après formation). On observe une baisse graphique du taux de prescription des hypnotiques (toutes classes confondues) avec un taux à 61,1% à quinze jours après formation, 56,3% à un mois de la formation et 58,2% à trois mois après formation contre 67,2% initialement, une semaine avant formation. Cette baisse n'est cependant pas statistiquement significative entre les quatre périodes. Après formation, on observe une baisse graphique de la prescription des benzodiazépines et des molécules apparentées aux benzodiazépines mais là encore cette baisse n'est pas statistiquement significative par rapport à la période de référence avant formation. Conclusion : après formation, le traitement intra-hospitalier de l'insomnie chez le sujet âgé n'était pas modifié : benzodiazépines et Z-drugs restaient trop souvent et trop longtemps prescrits. Il est indispensable de généraliser et de promouvoir l'accès de la thérapie cognitive et comportementale pour le traitement de l'insomnie. Il convient de réévaluer l'application de ces recommandations chez le sujet âgé régulièrement tant à l'hôpital qu'en ambulatoire.
