Elles ont une incidence majeure sur la santé des femmes et constituent un véritable problème de santé publique. Le médecin généraliste occupe une place prépondérante dans la chaîne de prise en charge de ces femmes, notamment au niveau de la détection des victimes. Le rôle du médecin dans ce dépistage est actuellement le sujet de nombreuses études et de multiples débats. Les objectifs de notre étude sont de connaître l'histoire des violences conjugales de chaque femme interrogée pour mieux en comprendre les fonctionnements, de recueillir le témoignage de femmes victimes concernant l'expérience vécue avec leur médecin généraliste et déterminer leurs avis et attentes sur le dépistage des violences conjugales de manière générale et lors d'une démarche systématique et enfin de faire émerger une ligne de conduite plus précise pour les médecins généralistes. Nous avons réalisé une enquête qualitative par entretiens semi-dirigés individuels entre mars et juin 2012. Notre échantillon a été formé à l'aide de femmes recrutées au sein d'associations venant en aide aux victimes de violences conjugales de l'Institut Médico-Légal du CHU de Nice et de notre environnement personnel. 37 femmes ont participé à l'étude. La principale information mise en évidence concerne l'avis globalement favorable au dépistage des violences conjugales par le médecin généraliste malgré l'existence de craintes et de réserves. Les victimes estiment que c'est au médecin d'aborder le sujet en premier pour les amener ensuite à se livrer. Le dépistage s'effectuera en s'adaptant à chacune d'elle. Aucune technique de repérage n'est préférée à une autre, même si les femmes ont tendance à privilégier le questionnement direct et la question détournée au questionnaire auto-administré. Cependant, les avis se révèlent très partagés concernant le dépistage systématique. Il s'avère représenter une démarche utile pour certaines, alors que pour d'autres il serait très mal accepté dans la population générale. Enfin, le rôle d'interlocuteur privilégié du médecin généraliste dans la prise en charge des violences conjugales est discuté. Des victimes avancent de nombreuses conditions pour que ce rôle soit tenu correctement (qualité d'écoute et de compassion, instauration d'une relation de confiance médecin/patient, formation adéquate du médecin) et certaines femmes préfèrent s'adresser à des personnes référentes autres que le médecin. Notre étude nous a permis de conclure que les femmes victimes attendent l'implication et l'aide du médecin généraliste. Elles sont quasiment univoques : c'est à lui de faire le premier pas pour amener la patiente à se livrer. Le dépistage devra être appliqué au cas par cas, notamment lors des périodes à risque de violences conjugales. Pour cela, il est indispensable que les médecins soient sensibilisés et fassent la démarche de se former.

Afin d’en déterminer les caractéristiques, et la place des médecins généralistes dans leur prise en charge, nous avons réalisé une enquête auprès des femmes victimes de violences conjugales de l’association SOS Femmes 93. Lors d’entretiens semi-directifs, nous avons questionné ces femmes sur les violences subies avant et pendant la grossesse, au moment de l’accouchement, et lors du retour au domicile. Certains questionnaires ont été complétés par des entretiens afin de préciser le rôle du médecin traitant auprès de ces femmes victimes. Sur les 28 femmes qui ont accepté de répondre au questionnaire, 27 ont été victimes de violences conjugales durant au moins une de leurs grossesses, et 100 % d’entre elles l’ont été en considérant la période du post-partum proche. Les violences sexuelles, les moins représentées, concernaient cependant 82 % d’entre elles. Sur les 62 grossesses étudiées, 11 se sont terminées par une fausse-couche (18 %), et 5 par une interruption volontaire de grossesse (8 %). Dix accouchements ont eu lieu prématurément sur les 44 grossesses menées à terme, soit 23 %. Trois accouchements sur 44 ont eu lieu à domicile (7 %). Pour seulement 18 des 62 grossesses étudiées, les femmes ont parlé des violences subies à un professionnel de santé (soit 29 % d’entre elles). La grossesse est une période à risque pour les femmes d’être victimes de violences conjugales. Les conséquences de ces violences sur les femmes et sur le fœtus sont une raison supplémentaire d’en améliorer le dépistage et la prise en charge, notamment par les médecins généralistes qui sont en première ligne.

He went on to write many books and film scripts.
