It is almost ten years since the last edition was written. In the intervening time, there have been numerous developments in cancer chemotherapy and in order to cover these the majority of the text has been completely revised and rewritten. In addition, two new chapters have been introduced, one on the safe handling of cytotoxic drugs and the other on biological response modifiers. In order to incorporate this new information without any undue increase in the length of the text the chapters on a combined approach to treatment have been omitted. This is not because interdisciplinary collaboration is no longer considered important but is a reflection of the fact that, in most centres, it has become the norm in cancer management and its central role in successful treatment no longer needs to be stressed quite so strongly. The four chapters in the last edition which dealt with the team approach to cancer therapy have been replaced by a single chapter on the place of chemotherapy in the overall treatment of cancer. Unfortunately, despite all the innovations of the last decade, it has become increasingly clear that much of the promise offered by drug treatment during the 1960s and 1970s has not been fulfilled.

However, the acquisition of drug resistance is a major clinical problem and is perhaps the main limiting factor in successful treatment of cancer. Thus, a tumor initially sensitive to chemotherapy will, in the majority of cases, eventually recur as a resistant tumor, which will then progress. Much of our understanding of drug resistance mechanisms comes from the study of tumor cell lines grown in tissue culture. We now understand many of the - lecular mechanisms that can lead to a cell acquiring resistance to antic- cer drugs; however, we still do not know which mechanism(s) are those most relevant to the problem of clinical drug resistance. Indeed, given that many of the cytotoxic anticancer drugs were discovered by random screening, it is - clear what features give a clinically useful anticancer drug a sufficient the- peutic index to be of value. The aim of Cytotoxic Drug Resistance Mechanisms is to provide pro- cols that are appropriate for examining the mechanisms of cellular resistance to anticancer cytotoxics in human tumor samples. Tumor cell lines have been enormously useful as experimental models of drug resistance mechanisms, however they have limitations and we need to address the relevance of such mechanisms in patients’ tumors. Examining drug resistance in tumors is much more problematic than in cell lines.

We now know that the bone marrow is an organ with rapid cell renewal in which maintenance of the steady-state depends on a high rate of cell production, and that the bone marrow toxicity of cytotoxic agents can be explained by the perturbation of such steady-state conditions. Experimental researchers have analysed in detail the effects of cytotoxic agents upon defined hematopoietic cell populations to define the mechanism of action and the location within the cell cycle at which cytotoxic drugs exert their cytocidal action. From such studies, a classification of cytotoxic agents according to their action upon cells during different phases of the cell cycle has been proposed [80-82]. Others have used the acute perturbation of the hematopoietic systems, caused by application of single high doses of cytotoxic agents, to analyse the patterns of depletion and reconstitution of the various hematopoietic compartments in an effort to shed light on the regulatory mechanisms that are responsible for the delicately balanced cell production of the hematopoietic cell renewal systems. A great deal of information on the structure of the hematopoietic systems has come from such studies, and kinetic parameters of these systems have been defined. To clinicians, the hematopoietic toxicity of antineoplastic agents has been an undesired though unavoidable side effect of almost all cytotoxic drugs introduced into clinical practice.
