Book excerpt: Colorectal cancer is the third-ranked neoplasm in order of incidence and mortality worldwide, and it is the second cause of cancer death in industrialized countries. Risk factors that increase the likelihood of developing colorectal cancer include diet, genetic mutations, alcohol abuse, smoking, and being over 50 years of age. In Canada, over 20,000 cases of colorectal cancer are diagnosed annually, highlighting the importance of early detection, diagnosis and treatment of the disease. Colorectal cancer can be screened using fecal occult blood testing (FOBT), double contrast barium enema (DCBE), sigmoidoscopy and colonoscopy. More than 90% of colorectal cancers arise from adenomatous polyps, which are abnormal growths of tissue projecting from the lining of the colon or rectum. Colonoscopy and flexible sigmoidoscopy are two procedures that are currently used to detect and locate colon polyps and carcinomas. Computed tomographic colonography (CTC), or virtual colonoscopy, is an imaging technique of the colon involving helical computed tomography and computer software to generate high-resolution two- and three-dimensional images of the inner surface of the colon. CTC is less invasive, requires less bowel preparation, does not require sedation or analgesia, takes less time, and can be performed in patients who have had incomplete colonoscopy or are contraindicated for colonoscopy. Disadvantages of CTC include high cost, high radiation dose, and less sensitivity and specificity than colonoscopy. A previous CADTH report published in 2008 compared the clinical- and cost-effectiveness of colonoscopy to sigmoidoscopy and CTC in the context of colorectal cancer screening. For the purposes of this guide, a patient registry is an organized system that uses observational study methods to collect uniform data (clinical and other) to evaluate specified outcomes for a population defined by a particular disease, condition, or exposure, and that serves one or more predetermined scientific, clinical, or policy purposes. A registry database is a file (or files) derived from the registry. Although registries can serve many purposes, this guide focuses on registries created for one or more of the following purposes: to describe the natural history of disease, to determine clinical effectiveness or cost-effectiveness of health care products and services, to measure or monitor safety and harm, and/or to measure quality of care. Registries are classified according to how their populations are defined. For example, product registries include patients who have been exposed to biopharmaceutical products or medical devices. Health services registries consist of patients who have had a common procedure, clinical encounter, or hospitalization. Disease or condition registries are defined by patients having the same diagnosis, such as cystic fibrosis or heart failure.
