This book, a completely revised second edition of our previous volume on the subject, maps out the nature of EU health policies, their logic and reason for being, and their potential to affect the health of Europeans for the better. It is written in the belief that understanding the breadth and diversity of EU health policies, and the distinctive institutional structure that explains them, will improve our collective abilities to make policy for health in any sphere, from food to healthcare services and from occupational safety to international trade. Above all, we hope that this book makes it impossible to deny the scale and often indirect and positive impact of EU health policy. It covers England France Germany Italy the Netherlands Slovenia Sweden Poland and the Republic of Moldova and aims to give insights into current practice that will support decision-makers in their efforts to strengthen public health capacities and services. Each country chapter captures the historical background of public health services and the context in which they operate; sets out the main organizational structures; assesses the sources of public health financing and how it is allocated; explains the training and employment of the public health workforce; and analyses existing frameworks for quality and performance assessment. The study reveals a wide range of experience and variation across Europe and clearly illustrates two fundamentally different approaches to public health services: integration with curative health services (as in Slovenia or Sweden) or organization and provision through a separate parallel structure (Republic of Moldova). The case studies explore the context that explain this divergence and its implications. This study is the result of close collaboration between the European Observatory on Health Systems and Policies and the WHO Regional Office for Europe Division of Health Systems and Public Health. These networks are meant to improve access to and provision of high-quality health care to all patients who have conditions requiring a concentration of specialized resources or expertise. At the same time they could act as focal points for medical training and research, information dissemination and evaluation, especially for rare diseases. The idea of pooling resources in order to better address medical conditions that are rare or require very specialized expertise or equipment, corresponds with moves towards concentration of specialized health care services, often motivated by common health systems challenges like tightening financial constraints, workforce shortages and growing attention for quality and safety. This book examines the different ways in which the concept of reference networks has been implemented in European countries, and what kind of medical conditions or interventions it covers in various countries. It also looks at the motivations behind the establishment of such networks, the regulatory and administrative processes for identifying and designating them, as well as the financial arrangements needed for their proper functioning. This study outlines the key policy implications and challenges for developing the concept of reference networks at national and European levels. Each of these crises has revealed profound splits in society, which are represented in terms of an opposition between those countries on the losing and those on the winning sides of globalisation. Inequalities beyond those between nations are critically absent from the debate. Based on major European statistical surveys, the new research in this work presents a map of social classes inspired by Pierre Bourdieu's sociology. It reveals the common features of the working class, the intermediate class and the privileged class in Europe. National features combine with social inequalities, through an account of the social distance between specific groups in nations in the North and in the countries of the South and East of Europe. The book ends with a reflection on the conditions that would be required for the emergence of a Europe-wide social movement.
