Book excerpt: Les implants sont considérés de gros diamètre quand ils sont compris entre 4.8 et 6 mm. Les indications pour poser des implants de grand diamètre sont multiples. Généralement, ils seront plutôt réservés aux secteurs molaires et éventuellement prémolaires en fonction des diamètres radiculaires des dents extraites correspondantes. En effet, les implants de grand diamètre permettent un meilleur profil d'émergence prothétique, particulièrement sur les sites molaires. Des considérations occlusales sont aussi à prendre en compte en fonction de la table occlusale des molaires et des prémolaires car les forces occlusales sont plus importantes sur les secteurs latéraux, notamment dans les mouvements de latéralité travaillant et non travaillant, les gros diamètres assurant ainsi une meilleure résistance à la fracture. Ils permettent éventuellement de remplacer une canine dont la largeur radiculaire est souvent plus importante (5 à 6 mm). On utilisera plus particulièrement ce type d'implant lorsque la hauteur osseuse résiduelle après extraction est réduite, en partant du principe que, pour favoriser l'ostéointégration et la stabilité des implants à long terme, on compensera une longueur courte par un plus grand diamètre pour avoir la même surface os/implant en contact. Dans le protocole d'extraction-implantation immédiate, les implants de gros diamètre permettent un meilleur ancrage latéral si des conditions anatomiques (nerf alvéolaire, sinus...) contre-indiquent un ancrage au-delà de l'apex. Enfin, ils peuvent être une solution de secours dans un os de mauvaise densité/qualité quand, par exemple, un implant de diamètre 4 mm a été posé avec une mauvaise stabilité primaire ou quand un implant de diamètre standard est fracturé et que l'on souhaite déposer et réimplanter immédiatement.

Book excerpt: Les implants de gros diamètre ont été proposés à la fin des années 80 pour améliorer le pronostic implantaire dans des situations osseuses peu favorables (qualité et/ou hauteur osseuse insuffisante). Ils permettent aussi une implantation immédiate après extraction, le remplacement immédiat d'un implant de diamètre standard non ostéointégré ou fracturé. La largeur crestale pour des implants larges doit être supérieure ou égale à 7 mm. Pour le remplacement des molaires unitaires maxillaires ou mandibulaires, il existe trois alternatives thérapeutiques : le remplacement par un implant de diamètre standard, par un implant large et par deux implants de diamètre standard. Le remplacement d'une molaire par un implant standard est une thérapeutique risquée sur le plan biomécanique à cause du risque de fracture de l'implant dû au porte-à-faux existant au niveau de la couronne. La thérapeutique à deux implants en remplacement d'une molaire est une solution très acceptable mais elle est limitée par la largeur mésio-distale. Les taux de succès à court et moyen terme sur le remplacement d'une molaire par un implant large sont globalement satisfaisants. Les implants de gros diamètre doivent être posés avec prudence dans un os très dense (type I), en privilégiant les séquences terminales manuelles.
