Risk factors include adenotonsillar hypertrophy, obesity, and craniofacial abnormalities (e.g. Down Syndrome, DS). The aim was to describe the craniofacial morphology in children with suspected OSA referred for polysomnography (PSG) in 2 cohorts: DS and obesity. Methods: Cross-sectional study of children with DS or obesity referred for PSG at SickKids, Toronto. Orthodontic examinations, PSG, lateral cephalograms, and sleep questionnaires were completed. Results: 42 children (20 DS, 22 obese) aged 5-18 (11.9 +3.6) were included. DS with OSA (Obstructive-Apnea-Hypopnea-Index (OAHI) >1) had increased palatal depth (p=0.04); OAHI was correlated with intercanine distance (r=0.48, p=0.03). These characteristics vary significantly among the literature and are limited by the infrequent use of polysomnogprahy, the gold standard for diagnosing and quantifying obstructive sleep apnea. Objective: To compare the prevalence of facial and/or dental imbalances in children with and without obstructive sleep apnea in cohorts of obese and non-obese children. Methods: A prospective, cross-sectional study of children (ages 4-16) who were referred for a polysomnogram at The Hospital for SickKids. Facial features and malocclusion was assessed clinically by one dentist, blinded to the PSG results. Results/Conclusions: Horizontal excess (overjet) was the only dentofacial finding which was significantly more common in children with obstructive sleep apnea as compared to those without obstructive sleep apnea (p=0.04). Dentofacial characteristics were also not different between children using positive airway pressure therapy and children not on positive airway pressure therapy.

Abstract: While obstructive sleep apnea among children with Down syndrome is very common, the pre-existing risk factors and its impact to neurodevelopment are not well known. The aims and objectives of this systematic review are to determine the prevalence of OSA among DS children and to identify the associated risk factors. 6 articles that met the inclusion criteria were retrieved after using PubMed and Google Scholar in literature searching. The prevalence of OSA has great variation among different countries but are relatively high, ranging from 57% to 79%. OSA was significant associated with obesity, age and tonsil size. It is also known to associate with behavioral problem in DS children and delay their neurodevelopment. Therefore, early treatment followed by appropriate modification in lifestyle and diet is crucial in managing OSA. Typical dental and craniomaxillofacial hereditary diseases refer to hereditary diseases which occurs in the oral and maxillofacial area, as well as systemic or other system hereditary diseases accompanied by oral abnormality. The former includes enamel hypoplasia, dentin hypoplasia and cleft lip and palate, which mainly occur in the oral cavity. While the latter commonly includes cranial clavicle hypoplasia, congenital ectodermal hypoplasia, osteogenesis imperfecta, and osteosclerosis. In addition, common diseases that appear locally in the oral cavity, including caries, periodontitis, oral mucosal diseases and head and neck tumors, are also considered to be related to genetic variations. With the increase of large-scale genome-wide association analysis study (GWAS) during the last decade, common loci for many genetic diseases have been identified. To date, most evidence comes from GWAS of dental caries and periodontal disease, which have tested associations between millions of single nucleotide polymorphisms (SNPs) and typical binary phenotypes. However, genetic loci for other rare oral diseases remain to be studied due to their large variety and low incidence. Multi-omics sequencing technology provides a more powerful tool for identifying the pathogenic genes of dental and craniomaxillofacial hereditary diseases at genomic, transcriptomic and proteomic level. In addition, with the development of single-cell multi-omics technology, specific cell populations involved in the pathogenesis of dental and craniomaxillofacial hereditary disease can be more accurately identified at single-cell resolution.
