Book excerpt: As the COVID-19 crisis began to take shape, all eyes were on Italy, the first Western country to attempt a response to the deadly pandemic. For institutional decision makers and average citizens alike, it was a time of deep uncertainty. As scientists struggled to understand the nature of the virus and how it spread, the gradualness with which information became available caused only deeper uncertainty, as did the inevitable disagreements over which protective actions the government should put in place. Despite some initial delay in its response, the Italian government eventually implemented a nationwide lockdown, which helped control the spread of the disease but simultaneously created unintended consequences for vulnerable populations, like small business owners, women, the elderly, and workers living paycheck to paycheck. Drawing on data surveys conducted during the transition between the first lockdown and staged reopening, this book examines people's risk perception and their willingness to trust the sources and channels of information that were available to them. It also looks at their attitudes toward the protective behaviors they were asked to adopt and the ways in which their own cultural worldviews impacted their support for pandemic response policies. With remarkable depth and candor, respondents reflected on what a post-COVID-19 Italy might look like, filling out the book with the hopes and fears of real people who had stared death in the face and lived to tell about it. The book looks ahead to possibilities for future research, policy, and practice. COVID-19 in Italy elaborates and tests several aspects of the Protective Action Decision Model (PADM) in the Italian context, introducing the concept of ontological security and insecurity as an explanatory change factor to help interpret the Italian experience of responding to COVID-19.

Book excerpt: Mapping the Epidemic: A Systemic Geography of COVID-19 in Italy provides a theoretical-methodological framework based on space-time analysis to map and interpret the set of factors that could have contributed to the spread of COVID-19, as well as a reflexive cartographic mapping visualizing the virus's dynamics. After an introduction that constitutes the theoretical anchor of the work carried out both with respect to territorial analysis and the use of reflexive cartography, the book discusses the role played by reflexive cartography in research on the COVID-19 pandemic conducted by an Italian university working group dealing with reticularity and the territorial fragilities that have influenced the spread. The data, subjected to analysis, are translated into reflexive cartography as a tool for restitution and investigation of the territorial dynamics. Each chapter consists of detailed information in which the European context of data analysis is illustrated, to then investigate the Italian territory and focus on the case of Lombardy and, in particular, of Bergamo as the epicenter. The book addresses the theoretical and methodological approaches of mapping the epidemic in Italy and the importance of cartography in the outbreak response, as well as including data accounting for contributing factors such as atmospheric pollution and infection rate, population distribution and major mobility corridors, and measures adopted to contain the outbreak, by implementing mapping at the regional Lombard, national, and European levels. Book excerpt: Italy was the first Western country to experience a major coronavirus outbreak and consequently faced large-scale health and socio-economic challenges. The Italian government enforced a wide set of homogeneous interventions nationally, despite the differing incidences of the virus throughout the country. Objective: The paper aims to analyse the policies implemented by the government and their impact on health and non-health outcomes considering both scaling-up and scaling-down interventions. Methods: To categorise the policy interventions, we rely on the comparative and conceptual framework developed by Moy et al. (2020). We investigate the impact of policies on the daily reported number of deaths, case fatality rate, confirmation rate, intensive care unit saturation, and financial and job market indicators across the three major geographical areas of Italy (North, Centre, and South). Qualitative and quantitative data are gathered from mixed sources: Italian national and regional institutions, National Health Research and international organisations. Our analysis contributes to the literature on the COVID-19 pandemic by comparing policy interventions and their outcomes. Results: Our findings suggest that the strictness and timing of containment and prevention measures played a prominent role in tackling the pandemic, both from a health and economic perspective. Technological interventions played a marginal role due to the inadequacy of protocols and the delay of their implementation. Conclusions: Future government interventions should be informed by evidence-based decision making to balance, the benefits arising from the timing and stringency of the interventions against the adverse social and economic cost, both in the short and long term.

Countries across Europe understand what they are or what they should include differently. This study describes the experiences of nine countries detailing the ways they have opted to organize and finance public health services and train and employ their public health workforce. It covers England France Germany Italy the Netherlands Slovenia Sweden Poland and the Republic of Moldova and aims to give insights into current practice that will support decision-makers in their efforts to strengthen public health capacities and services. Each country chapter captures the historical background of public health services and the context in which they operate; sets out the main organizational structures; assesses the sources of public health financing and how it is allocated; explains the training and employment of the public health workforce; and analyses existing frameworks for quality and performance assessment. The study reveals a wide range of experience and variation across Europe and clearly illustrates two fundamentally different approaches to public health services: integration with curative health services (as in Slovenia or Sweden) or organization and provision through a separate parallel structure (Republic of Moldova). The case studies explore the context that explain this divergence and its implications. Book excerpt: In recent public workshops and working group meetings, the Forum on Microbial Threats of the Institute of Medicine (IOM) has examined a variety of infectious disease outbreaks with pandemic potential, including those caused by influenza (IOM, 2005) and severe acute respiratory syndrome (SARS) (IOM, 2004). Particular attention has been paid to the potential pandemic threat posed by the H5N1 strain of avian influenza, which is now endemic in many Southeast Asian bird populations. Since 2003, the H5N1 subtype of avian influenza has caused 185 confirmed human deaths in 11 countries, including some cases of viral transmission from human to human (WHO, 2007). But as worrisome as these developments are, at least they are caused by known pathogens. The next pandemic could well be caused by the emergence of a microbe that is still unknown, much as happened in the 1980s with the emergence of the human immunodeficiency virus (HIV) and in 2003 with the appearance of the SARS coronavirus. Previous Forum meetings on pandemic disease have discussed the scientific and logistical challenges associated with pandemic disease recognition, identification, and response. Participants in these earlier meetings also recognized the difficulty of implementing disease control strategies effectively.
