There is a large demand of high-quality education, information and training in this field and we answered that by organising the COVID19 in Pregnancy and Childhood Days: this book represents the hard legacy of this event with the contribution of key opinion leaders in all the aforementioned specialties. The book is answering a clear need and is directed to: 1) obstetricians, 2) neonatologists, 3) pediatricians, 3) public health specialists, 4) adult critical care physicians, 5) infectious disease specialists, 5) psychologists. Last but not least, the informations resumed in the book are going to be useful for healthcare authorities to program perinatal care and improve it. By 1978, it had become evident that the coronavirus genomic RNA was infectious (i. e. , positive strand), and by 1983, at least the framework of the coronavirus replication strategy had been per ceived. Subsequently, with the application of recombinant DNA techniques, there have been remarkable advances in our understanding of the molecular biology of coronaviruses, and a mass of structural data concerning coronavirus genomes, mRNAs, and pro teins now exists. More recently, attention has been focused on the role of essential and accessory gene products in the coronavirus replication cyde and a molecular analysis of the structure-function relation ships of coronavirus proteins. Nevertheless, there are still large gaps in our knowledge, for instance, in areas such as the genesis of coronavirus subgenomic mRNAs or the function of the coronavirus RNA-dependent RNA polymerase. The diseases caused by coronaviruses have been known for much longer than the agents themselves. Possibly the first coronavirus-related disease to be recorded was feline infectious peritonitis, as early as 1912. The diseases associ ated with infectious bronchitis virus, transmissible gastroenteritis virus, and murine hepatitis virus were all well known before 1950.

The authors argue that four virtues -- self effacement, self sacrifice, compassion, and integrity -- generate the physician's obligation to protect and promote the patient's interest. They then identify the three types of patient's interests -- social role interests, subjective interests, and deliberative interests -- and they reinterpret the ethical principles of beneficence and respect for autonomy in terms of these. The concept of the fetus as patient, the physician's obligation to third parties, and the moral standing of fathers and family members are also addressed. The implications of their argument sets the stage for the discussions of prevention and management in the remaining sections of the book. Ethics in Obstetrics and Gynecology is a unique addition to the literature in both biomedical ethics and obstetrics and gynecology. It demonstrates that ethics should be regarded as an essential part of obstetrics and gynecology, and that clinical practice is incomplete without it.

The committee recommends that testing HIV be a routine part of prenatal care, and that health care providers notify women that HIV testing is part of the usual array of prenatal tests and that they have an opportunity to refuse the HIV test. Book excerpt: "Preeclampsia is becoming an increasingly common diagnosis in the developed world and remains a high cause of maternal and fetal morbidity and mortality in the developing world. It is a multisystem progressive disorder of pregnancy characterized by the new onset of hypertension and proteinuria of 300 mg/day or of hypertension and significant end-organ dysfunction with or without proteinuria. It also presents with a variety of organ failures, including malfunction of kidneys, liver and lungs. It is estimated that about 5-7% of pregnancies are complicated by preeclampsia. This disorder always resolves after delivery. In about 80% of cases, pregnancy ends with good maternal and fetal outcomes. However, these pregnancies are still at increased risk for maternal and/or fetal mortality or serious morbidity. The remaining 20% of cases are at risk for preterm delivery and small gestational age infant. As regards the long-term consequences, women with preeclampsia are at high risk for cardiovascular, renal, and chronic hypertensive diseases. The exact pathophysiology of preeclampsia is poorly understood and, consequently, there are no well-established methods of primary prevention or of cost-effective screening. Childbirth is the only known cure; however, the decision between delivery and expectant management depends on several factors, such as fetal gestational age, maternal and fetal status at time of initial evaluation, presence of labor or rupture of fetal membranes, and level of available neonatal and maternal services. The aim of this book is to provide an overview of the latest developments about the physiopathology, diagnosis, and treatment of pre-eclampsia.
