Book excerpt: he 2010 Indian Health Service (IHS) oral health basic screening survey (BSS) of oral health status for American Indian and Alaska Native (AI/AN) children ages 1 through 5 years nationwide found substantial differences in caries prevalence between different IHS service areas. The reasons for these differences are not well understood. Socioeconomic status (SES) may be a related factor, as it is well established that SES is a strong predictor of caries prevalence. However, it is possible that some IHS service areas have managed to achieve relatively low caries prevalence despite low SES. This observational study examined associations between caries prevalence among preschool-aged AI/AN children and county-level SES to identify service areas in which caries prevalence was not well explained by SES.Two sources of data were used for this study. Caries prevalence data among children ages 1 through 5 years from 76 study sites was drawn from the 2010 IHS BSS. Demographic, economic and social information describing the AI/AN population in the county for each of the 76 sites were selected from the American Community Survey. Associations between county SES characteristics and study site caries prevalence were examined using correlation and linear regression analyses. Of the 76 study sites evaluated during the 2010 IHS BSS, caries outcomes and SES indicators varied widely. The percentage of children having experienced dental decay ranged from 14% to 88%, and percentage of children with untreated decay ranged from 0.5% to 74%. Percentage of decayed teeth overall ranged from 2.7% to 37.9%. Median annual income of the county ranged from $17,778 to $63,000; unemployment ranged from 6.4% to 32.9%, and percentage of the AI/AN population with a bachelor's degree ranged from 3% to 22.5%. Median income, unemployment, and educational all showed some correlation with caries prevalence. Linear regression analysis found that of the SES indicators studied, the SES indicator most predictive of caries prevalence was the percentage of the population having attained a bachelor's degree or a higher level of college education (part R2 11% - 22%). Multivariate linear regression analysis adjusting for median income, % unemployment, and % bachelor's degree found that these three SES indicators taken together can explain 29% of the variation observed in percent of teeth with decay, 21% of the variation in any decay experience, and 16% of the variation in any untreated decay. For many study sites, caries prevalence was much higher or lower than what would be predicted based on SES indicators. Sites with the highest and lowest observed caries prevalence tended to also be the sites with highest and lowest observed-predicted values: the healthiest and least healthy areas remained the healthiest and least healthy even after accounting for SES. Caries prevalence within the IHS BSS communities studied was not always well explained by SES, indicating that other factors may be contributing to caries prevalence. These communities should be studied further to discover what factors may be more related to caries prevalence.

Each mother was first given the structured questionnaire, and then the child s clinical dental examination was carried out. The prevalence of early childhood caries in preschool children aged three to five years in Bhopal was 43.2%, with a mean dmft of 3.02. Caries prevalence, however, showed statistically significant association and a negative correlation with socioeconomic status, with lower socioeconomic status children showing higher caries.

A total of 813 children aged 2-6 years were screened for the present study from randomly selected three kindergarten schools each from Government, Government aided, and private managements. Clinical examination was done inside the respective schools. At the time of examination, a proforma was filled for each child comprising of DFS index. The questionnaire by Winter et al. was modified and used .The completed proformas were statistically analyzed to find if any correlation existed between the nursing caries to the feeding practices and socioeconomic status of the family. There is a strong and significant relationship between the severity of nursing caries and the degree of feeding abuse. Children from low socioeconomic status have increased early childhood caries.Nursing caries were more in children who were taking a feeding bottle to bed at night and were increasingly seen in large families and lower socioeconomic groups.

The association of diet, food insecurity and caries is unknown. Objectives . To evaluate the associations of diet quality, food insecurity and caries prevalence and severity in children ages 2-11. To examine consistencies in associations between dietary quality indicators and dental caries status in young children in the National Health and Nutrition Examination Survey (NHANES). Methods . Children ages 2-5 (n=801) and 6-11 years (n=1097) from the 2001-2002 who had dental exam, food security, and diet data were studied. Logistic and linear regression was used to explore relationships between caries status and diet exposures and food security status, controlling for age, race, poverty, household smoking, dental visits and health insurance coverage. Diet quality indicators and dental caries status were compared between NHANES 2001-2002 and NHANES III [1988-1994] (n=4119). Results . Children ages 2-5 in the upper quartile of diet quality were 69% less likely to have early childhood caries (ECC) (OR 0.31, 95% CI 0.18, 0.56) compared with those in the lowest quartile. Young children living in food insecure households had higher caries prevalence than food secure children (38.2% vs. 23.0%, p =0.004) and a 1.8 greater odds of caries (95% CI 1.09, 2.97).
