Book excerpt: Since the introduction of coronary angioplasty in 1977, this procedure has gained a steadily increasing position in the treatment of coronary artery obstmction. From the available evidence it can be estimated, that this thera peutic tool will get even more additional momentum of many ten-thousands of patients to be treated in the next few years, due to a growing fraction of patients who are candidates for this intervention. Information about the indications, benefits and risks of coronary angioplasty is accumulating rapidly in addition to publications about refinements of the technique itself. Recently, a number of investigators have realized that coronary angioplasty is not only a therapeutic tool, but can, during the procedure, be used as a source of diagnostic information. When the catheter is placed in a coronary artery obstruction, inflation of the balloon produces transient myocardial ischemia. Before, during, and after this period of severe ischemia, studies of the perfor mance of the myocardium at risk can be carried out. The fact that therapeutic coronary angioplasty is carried out in a cardiac catheterization laboratory which is by definition optimally equipped for the measurement of hemodynamic parameters, has probably also contributed to the effectuation of these investigations. The combination of hemodynamic and biochemical parameters with morphological information from the coronary angiogram can be utilized for the quantification of myocardial involvement and the success of coronary dilatation with angioplasty. Studies of interactions with pharmacological substances are also feasible and informative.

Fundamentally, the answer to these questions can be given to a large extent by measuring coronary pressure. That is the rationale of writing this book. 1. 1 Historical overview. Andreas Gruentzig and most interventional cardiologists in the early days of PTCA, had the intuitive feeling that pressure measurements could help to establish the severity of a coronary stenosis and to monitor the progress and result of a coronary intervention. At that time, measuring coronary pressure by the balloon catheter was part of a standard procedure. A residual transstenotic gradient of less than 15 mmHg was generally considered as a good result. Later, however, it turned out that measuring these (resting) gradients with balloon catheters was inaccurate an only had a limited prognostic value. Moreover, because there was no consistent theory to correlate pressure measurements to blood flow, the interest in measuring coronary pressures faded and disappeared almost completely with the introduction of new balloon catheters not intended for pressure measurement.

Descriptions of aortic regurgitation had previously been published by others, but Corrigan's contribution was so comprehensive that his name is still closely associated with this disorder. He described the physical findings and the underlying gross pathologic anatomy of aortic regurgitation. He recognized that sudden death was not characteristic of aortic regurgitation, as it was in aortic stenosis, and his therapeutic approach was based on firm physiologic principles. In the past 150 years we have expanded Corrigan's work, and we have developed a detailed appreciation of the natural history, pathophysiology, diagnostic methods, and treatment of chronic aortic regurgitation. Fifteen years ago, cardiac catheterization and angiography had already achieved widespread application in the evaluation of aortic regurgitation, but cardiac ultrasound, especially Doppler echocardiography, was in its infancy, and the utility of radionuclide ventriculography was not widely appreciated.

A copious amount of representative figures and illustrations is included so that the reader is able to understand the clinical application of each modality in the various cardiovascular pathologies and the echocardiographic diagnosis. In keeping with the concept of an integrated echocardiographic examination, the two-dimensional, M-mode and Doppler echocardiographic findings are included in many of the diseases. Usually, the echocardiographic examination is approached first by two-dimensional imag ing to give an understanding of the anatomical correlations, if necessary adding M-mode to clarify the time course of intracardiac movements, and secondly by each Doppler technique to evaluate the hemodynamic conditions.
