Book excerpt: This report considers the biological and behavioral mechanisms that may underlie the pathogenicity of tobacco smoke. Many Surgeon General's reports have considered research findings on mechanisms in assessing the biological plausibility of associations observed in epidemiologic studies. Mechanisms of disease are important because they may provide plausibility, which is one of the guideline criteria for assessing evidence on causation. This report specifically reviews the evidence on the potential mechanisms by which smoking causes diseases and considers whether a mechanism is likely to be operative in the production of human disease by tobacco smoke. This evidence is relevant to understanding how smoking causes disease, to identifying those who may be particularly susceptible, and to assessing the potential risks of tobacco products.

Recent years have seen the publication of many textbooks on cardiac arrhythmias, some of which concentrate on one particular aspect such as drug management, electrocardiographic appearances, electrophysiological evaluation etc; and others of which are the collated reports of symposia, often dealing with detailed con siderations of highly specialised problems. Most of the larger more comprehensive texts have devoted a substantial proportion to basic considerations and experimental observations, far removed from the clinical arena. When asked to contribute to the series 'Current Status of Clinical Cardiology' we felt that the clinical aspects of cardiac arrhyth mias should be emphasized, and that the text should be as comprehensive as possible within the limitations of a single volume in this series. This comprehensive but clinical approach has necessitated the inclusion of certain subjects such as the mechanisms of tachycardia, metabolic aspects of cardiac arrhythmias and reperfusion arrhythmias, which are not directly or exclusively clinical. However, the rapid advances in these areas in recent years are likely to have increasingly important clinical con seq uences. Although the epidemiology of clinical arrhythmias is difficult to dis cover, it is widely appreciated that arrhythmias are commonplace.

The organic chemist who makes it is guided not only by the structure-action relations of previous compounds, but by anticipation of a requirement for a particular type of action. In fact several of the best-known antiarrhythmics, including lidocaine, mexiletine, amiodarone and verapamil, were originally synthesized for other purposes. Physicians have to determine whether the new drug works, and pharma cologists how it works. For some years I have believed that there was room for a work which could be understood by all these groups and which could enlighten each about the point of view of the others. Thus when I was invited by Springer-Verlag to prepare a volume in their series Handbook of Experimental Pharmacology, I already had a firm conception of what its form should be. In any multi-author work there are two objectives which cannot always readily be reconciled. The first is to select topics which would relate to each other in a coherent manner. to give a logical and orderly shape to the volume as a whole. The second is to offer authors the greatest possible freedom to express themselves as they wish. When the general design was complete, prospective contributors were invited to write specific chapters, being provided with a complete list of their coauthors and chosen topics, so that they could avoid overlap.

Many patients have survived very serious illness only because of intravenous nutrition for either short or very long periods of time. The indications for parenteral nutrition are simple and can be summarised as the inability to ingest necessary nutrients for a significant time during increased metabolic demands. Many problems in the preparation of energy sources and amino acid solutions have been solved so that the time is opportune to review what has been achieved and discuss recent advances and current thinking in the light of future requirements. The next phase in parenteral nutrition will undoubtedly be the provision of regimens designed for specific situations. The needs of the neonate for example are known to differ from adult requirements. The choice of carbohydrate for intravenous use has been a matter of much of glucose both in biochemical and clinical terms discussion. The supremacy now seems well established. The value of intravenous fat is well documented, but the interchange of fat and carbohydrate as calorie sources and the effects of prolonged infusions of fat merit further investigation. The evidence that isotonic amino acids are utilized effectively when given alone due to the availability of endogenous energy sources has led to a greater understanding of the metabolic changes and demands associated with injury and sepsis.
