Unnecessary, because there is compelling evidence, as this Guide makes clear, that cervical cancer is one of the most preventable and treatable forms of cancer, as long as it is detected early and managed effectively. Unfortunately, the majority of women in developing countries still do not have access to cervical cancer prevention programmes. The consequence is that, often, cervical cancer is not detected until it is too late to be cured. An urgent effort is required if this situation is to be corrected. This Guide is intended to help those responsible for providing services aimed at reducing the burden posed by cervical cancer for women, communities and health systems. It focuses on the knowledge and skills needed by health care providers, at different levels of care.

Even as the professionalism of medicine progressed, many sufferers continued to rely on what would now be termed "fringe" practitioners - quacks, backstreet surgeons, bone-setters, Thomsonian botanists, holists and naturalists. Many types of fringe medicine were popular in particular circles or reflected the political or religious preoccupations of their practitioners. Anti-establishment radicals might favour natural medicine, Christian Scientists would reject the medical aid, "Physical Puritans" would concentrate on homeopathy, hydropathy and vegetarianism to create health rather than counter disease. Some diseases, particularly venereal ones, allowed practitioners to play unscrupulously on the guilt of their patients. The end of the period saw professionalism establish itself in many areas, for example with the foundation in 1852 of the Pharmaceutical Society, and conflicts of fringe and orthodoxy became the fiercer. The essays collected in this volume all present new research on this fascinating and diverse period in the history of medicine.

Drawing on newspaper accounts, medical and pharmacy journals, professional association files, asylum documents, physicians’ case books, and pharmacy records, Malleck demonstrates how a number of social, economic, and cultural forces converged in the early 1900s to influence lawmakers and criminalize addiction. His research exposes how social concerns about drug addiction had less to do with the long pipe and shadowy den than with lobbying by medical professionals, a growing pharmaceutical industry, and concern about the morality and future of the nation.

Without the technological, industrial, intellectual, organisational and research-related capabilities associated with competent pharmaceutical production, and without policies that pull the industrial sectors towards serving local health needs, the African sub-continent cannot generate the resources to tackle its populations' needs and demands. These guidelines address persisting pain in children caused by conditions such as cancer, HIV/AIDS, sickle-cell disease, burns, trauma, and phantom limb pain. Recommendations -- developed following a careful and transparent appraisal of available evidence -- are presented for the pharmacological treatment of mild, moderate and severe pain. The guidelines include chapters on the various systems used to classify pain and on the evaluation of pain, which reviews the available tools for routine pain measurement in children. Provision of sustainable pain relief within health care systems is covered in a separate chapter. Since morphine and other opioid analgesics -- required for the relief of moderate-to-severe pain in children -- are listed under the 1961 UN Single Convention on Narcotic Drugs, the new guidelines include an annex explaining the Convention's requirements for the handling and procurement of opioid analgesics for the relief of pain.

Book excerpt: Countries that have made the most progress toward universal coverage have public expenditures in health equivalent to at least 6% of their gross domestic product (GDP), which is the percentage established in PAHO's universal health strategy as the benchmark for the countries. However, while higher expenditure is a prerequisite, it is not enough to combat inequities and advance toward universal health. In addition to greater resources, the quality of the expenditure must be improved, reducing health system inefficiencies. Moreover, public expenditure in health should be sustainably increased in a fiscally responsible manner. The concept of fiscal space for health refers to the ability of governments to provide additional budgetary resources for the health system without affecting the financial position of the public sector or supplanting other socially necessary expenditures. Any analysis of fiscal space, therefore, will attempt to identify the prospects for increasing health expenditure in the short and medium term to address a series of clearly established health needs. These efforts are under way at a critical time in the Region of the Americas, particularly in the countries of Latin America and the Caribbean, which are engaged in a singular health system reform process. For the first time in history, these countries have formalized their intention of increasing public expenditure in health, putting themselves firmly on the path to real and effective access to health care through the universal health strategy. Without achieving basic well-being at this level, it will be impossible to improve social cohesion and social development in the countries of the Region. This publication brings together and summarizes PAHO's studies on fiscal space for universal health in the Americas and draws on the contributions of the regional forum held in Washington, D.C. on 7-8 December 2015. With this publication, whose target audience is the technical personnel responsible for policy development, decision-makers, and authorities, PAHO hopes to contribute to the analysis and discussion of health financing policies on the path toward universal health.
