Pour cela, nous décrirons la mastication, les muscles, l'occlusion et l'articulation temporo-mandibulaire. Enfin nous essayerons de prévoir une étude centrée sur la mastication et la présence de contacts dentaires, ce qui pourrait déclencher ou réactiver des DTM en cas de malocclusion.

Afin d'améliorer leur prise en charge, les connaissances sur l'articulation temporomandibulaires et ses interrelations avec le reste du corps sont indispensables. A ce jour, le lien entre l'occlusion et la posture fait l'objet de grands débats ; en dépit de la multiplicité des études à ce sujet, aucun consensus n'est apparu dans la littérature scientifique. La posture, état d'équilibre du corps dans son environnement, est maintenue grâce aux différentes informations apportées par le système visuel, vestibulaire et le système proprioceptif périphérique. Qu'en est-il de l'articulation temporo-mandibulaire et de l'occlusion ? Objectifs : Déterminer à travers une étude préliminaire si une perturbation de l'occlusion est à l'origine d'une perturbation de la posture. Matériel et méthode : Une perturbation de l'occlusion est matérialisée à travers le port d'une cale occlusale mandibulaire. Les sujets, sans le port de la cale occlusale puis avec, ont réalisé deux tâches afin d'évaluer l'évolution de leur stabilité posturale durant : - L'initiation à la marche - Le maintien en appui unipodal. Les données ont été récoltées à l'aide d'un système opto-électronique synchronisé à une plateforme de force. Résultats et conclusion : Le protocole réalisé n'a pas permis de valider l'hypothèse de départ. Le port du dispositif a permis de stabiliser la posture des sujets durant les différentes taches effectuées. Cette étude a montré ses limites. La méthodologie appliquée dans ce travail a révélé un biais important ; celui de la double tâche. Une étude à plus grande échelle, prenant en compte les pistes d'amélioration décelées dans cette étude est désormais nécessaire pour démontrer les relations entre l'occlusion et la posture.

Neuroscience has been able to shed some light on the multiple connections between trigeminal and non-trigeminal nervous centres, which confirms the significant involvement of the stomatognathic system and trigeminal nerves (V) in both oral as well as non-oral major functions such as eating, breathing, speaking, hearing, and standing ... and also confirms the extensive participation of the paired Vs in the human adaptation process. Too many physicians are still reluctant to admit the pathological responsibilities of the Vs. Therefore their role remains largely underestimated by clinicians. Stress conditions in introverted people cause a parafunctional habit (i.e. severe clenching), which in turn produces trigeminal overstimulation and nociception. Through a process of sensitization this can perturb some non-trigeminal nervous areas, such as the vestibular nuclei and cerebellum (involved in equilibrium). Would you then be willing to accept the possibility that a stressed and introverted patient who clenches hard, durably or frequently may end up suffering from dizziness? If not, this book is not for you.
