Nous nous intéresserons au travers de cette thèse à l’étude de ce thème au sein de la profession de chirurgien-dentiste qui est celle présentant le taux d’augmentation de professionnels à diplôme étranger le plus élevé ces dernières années. Quand est-il des chirurgiens-dentistes faisant le choix de quitter la France pour exercer dans un autre pays ? Quelles en sont les raisons et doit-on s’en inquiéter face à l’arrivée massive de professionnels à diplôme étranger ? Faisons le point sur la démographie actuelle de notre profession en France. Quelle évolution pour notre pays ?

En Ile-de-France, la densité des chirurgiens-dentistes est élevée mais il existe une mauvaise répartition puisque les praticiens se concentrent dans les grandes agglomérations en défaveur des zones rurales. Le gouvernement a donc mis en place en 2009 la loi Hôpital, Patients, Santé et Territoire (HPST) qui entend lutter contre les inégalités territoriales avec la mise en œuvre de plusieurs mesures : le déploiement de Centres de santé et Maisons de santé sur le territoire national en particulier en milieu rural, et la création d'un Contrat d'Engagement au Service Public. Le CESP est une aide versée aux étudiants s'engageant à travailler dans des zones sous-dotées en chirurgiens-dentistes pendant une durée équivalente au bénéfice de l'aide. La première partie de ce travail permet de décrire l'évolution de la démographie professionnelle des chirurgiens-dentistes, ainsi que les mesures de régulation et l'état des lieux en région Ile-de-France. La seconde partie est consacrée à une étude sur l'exercice des chirurgiens-dentistes diplômés à l'UFR d'Odontologie de l'Université Paris Diderot depuis les cinq dernières années afin de connaître leurs parcours et leurs choix professionnels.

It is now widely recognized that human resources for health (HRH) are a key enabler for the attainment of universal health coverage, and for the achievement of Sustainable Development Goal (SDG) 3 - Ensure healthy lives and promote well-being for all at all ages. As is stressed in the Global Strategy on Human Resources for Health: Workforce 2030, there can be no viable national, or global, health system without an effective health workforce. The Global Strategy, adopted at the Sixty-ninth World Health Assembly in May 2016, challenges the erroneous narrative of health workers as a unit of cost in the production of health. The evidence instead presents an intersectoral agenda on the pre-condition of equitable access to health workers in the attainment of universal health coverage, along with a dynamic labour market understanding of the substantive impact on education, employment, jobs and innovation in the health and social care economy. The Global Strategy, therefore, enables governments and other relevant stakeholders to adopt a holistic, rather than fragmented, approach to ensuring that the health workforce contributes both to improved health and to broader socioeconomic development.
