Seules trois études, présentant des résultats divergents, se sont intéressées au lien entre consommation chronique de benzodiazépines et déclin cognitif. L'objectif de notre étude était de déterminer si la consommation chronique de benzodiazépine est associée à une accélération du déclin cognitif avec l'âge, chez les sujets de plus de 65 ans. Notre étude a porté sur 1 449 sujets de plus de 65 ans issus de la cohorte des trois cités, recrutées dans les villes de Bordeaux et Montpellier, en France, entre 1999 et 2001, sur la base d'un tirage au sort sur la liste électorale. Les données cognitives et celles concernant la consommation de benzodiazépine des sujets ont été colligées à l'inclusion, puis lors des vistes de suivis à 2, 4 et 7 ans. 1 282 consommateurs chroniques ayant présenté une reprise de benzodiazépine à chacune des quatre visites ont été comparés à 167 sujets non consommateurs n'ayant présentés aucune prise de benzodiazépine lors des visites de suivi. Le déclin cognitif a été analysé à l'aide d'un modèle multivarié non linéaire à variable latente pour données longitudinales. Les résulats de 5 tests neuropsychologiques ont été utilisés: le Mini Mental state Examination, l'Isaacs set test, les test de rétention visuelle de Benton, et les deux parties A et B du Trail Making Test. La consommation chronique de benzodiazépines était associée à un bas niveau cognitif (ß=0,024 p=0,006). Aucune association n'était retrouvée entre cette consommation et la vitesse du déclin cognitif (ß*âge = 0,005 pvalue=0,445). Nos résulats suggèrent que la consommation de benzodiazépines n'est pas le facteur de risque de déclin cognitif avec l'âge

Au 1er janvier 2015, la classe des 65 ans et plus représente plus de 12,3 millions d'habitants parmi les 66 millions d'habitants que compte la France. De plus, 11,5 millions de français ont consommé au moins une benzodiazépine et environ 20% des 12 millions de personnes âgées consomment de façon chronique des hypnotiques ou anxiolytiques. Les conditions physiologiques normales de l'âge les rendent naturellement plus vulnérables aux risques liés à l'utilisation de ces molécules. Ainsi, les chutes, les accidents de la route et les troubles cognitifs sont des cas fréquemment rencontrés. En effet, la consommation de benzodiazépines chez le sujet âgé a un retentissement non négligeable sur le fonctionnement mnésique. Plusieurs mécanismes biochimiques sont impliqués mais tous les types et stades de mémoires ne semblent pas être atteints de la même manière. De plus, des études récentes montrent un lien probable entre la consommation des benzodiazépines et la maladie d'Alzheimer mais il convient de rester prudent sur les données obtenues car aucune certitude n'a encore été établie. Enfin, du fait de l'importance de leur consommation en France, des effets indésirables non négligeables qu'elles génèrent et de certains usages problématiques, les benzodiazépines font l'objet, depuis de nombreuses années, d'une attention particulière des autorités sanitaires en particulier de l'ANSM.

Book excerpt: La France est un des pays où la consommation de psychotropes et notamment celle de benzodiazépines (BZD) est la plus élevée. Chez les sujets âgés, l’utilisation au long cours de BZD est associée à un risque accru de présenter différents événements graves, notamment un risque accru de démence. Le mécanisme expliquant cette association n’est pas élucidé, et l’effet des BZD sur les performances cognitives, en particulier à long-terme, reste controversé. L’objectif principal de cette étude était d’étudier l’association entre l’exposition incidente aux BZD et l’évolution du déclin cognitif chez des sujets initialement non déments. La population d’étude était issue de la cohorte PAQUID, réalisée chez des sujets de 65 ans et plus résidant en Dordogne et Gironde et offrant jusqu’à 20 ans de suivi. Parmi les sujets non utilisateurs de BZD à l’inclusion et non déments au suivi à 3 ans (T3) dans la cohorte PAQUID, 102 exposés incidents aux BZD et 715 non exposés à T3 ont été identifiés. L’évolution de leurs fonctions cognitives a été étudiée en considérant les résultats obtenus au Mini Mental Status Examination (MMSE) et à l’Isaac Set Test (IST) aux visites de suivi réalisées tous les 2-3 ans après T3 (T5 à T20). L’association entre l’exposition incidente aux BZD et l’évolution des performances cognitives a été étudiée à l’aide de modèles non linéaires mixtes considérant un processus latent comme variable à expliquer. Des analyses secondaires portant sur l’évolution de la symptomatologie dépressive, des troubles du sommeil et de l’anxiété ont été réalisées afin d’étudier un éventuel biais protopathique. L’exposition incidente aux BZD n’était pas associée à une différence d’évolution des performances cognitives des sujets évaluées au regard du MMSE ou de l’IST, ni à court-terme ni au long-terme. L’analyse secondaire n’a pas mis en évidence de différence d’évolution de la symptomatologie dépressive, des troubles du sommeil ou de l’anxiété, qui aurait pu témoigner d’un possible biais protopathique dans l’association entre BZD et démence. Indépendamment du risque de démence, l’initiation après 65 ans d’un traitement par BZD ne paraissait pas associée à un risque accru de dégradation des performances cognitives à long terme.

It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? What are the possible links between such tools and social networks? Is there a gap between evidence-based results and market development? Are there any quality challenges? What future developments and studies are needed in the field?
