Book excerpt: Contexte : La France est l’un des pays ou la consommation de médicaments psychotropes est la plus élevée, en particulier pour les benzodiazépines. L’AFSSAPS tire la sonnette dans son rapport d’expertise de janvier 2012 sur cette surconsommation. Les facilités de renouvellement, le temps de consultation trop court, la pression du patient et le manque d’informations des patients sont des facteurs parmi d’autres ne permettant pas de diminuer cette surconsommation. La connaissance des patients des risques liées aux benzodiazépines influe-t-elle sur leur consommation ? Méthode : Etudecre prospective descriptive sous forme d’un questionnaire anonyme concernant les connaissances des risques des benzodiazépines des patients et description de la consommation rempli par un médecin investigateur suivi d’une information complémentaire sur les risques de la consommation de benzodiazépines au long cours avec proposition de sevrage des benzodiazépines. Evaluation de l’état des connaissances des patients consommateurs et non consommateurs des risques des BZD et du mode de consommation et de l’impact de cette intervention sur la consommation de BZD à 3 et 6 mois. Résultats: Les effets secondaires sont peu connus des patients quelque soit leur niveau socio intellectuel. 14 % des patients consommateurs pensent qu’il n’y a aucun risque à consommer de manière chronique des benzodiazépines. A 6 mois, 61% des patients ont arrêter ou ont diminué de plus de 25% leur consommation de benzodiazépines. Discussion : La prévalence de la consommation de benzodiazépine est trop élevée. Les patients sont mal informés. L’information complète permet de diminuer la consommation de benzodiazépines. Différentes mesures réglementaires ont été engagées durant ces dernières années mais l’information du patient n’a pas assez été soulevée. Notre étude interventionnelle a permis de souligner l’importance de l’information des patients des risques et de la nécessité de mettre en place un contrat entre le patient et le médecin lors de la prescription.

Book excerpt: Recent years have seen important developments in the computer and game industry, including the emergence of the concept of serious games. It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? What are the possible links between such tools and social networks? Is there a gap between evidence-based results and market development? Are there any quality challenges? What future developments and studies are needed in the field?

In 1993, the first version of these guidelines, titled: ‘An outline for the management of alcohol problems: Quality assurance in the treatment of drug dependence project’ was published (Mattick & Jarvis 1993). The Australian Government commissioned an update a decade later (Shand et al. 2003) and a further edition in 2009 to integrate the Guidelines with the Australian Guidelines to Reduce Health Risks from Drinking Alcohol (National Health and Medical Research Council, NHMRC 2009; Haber et al., 2009). The present version of the Guidelines was also commissioned by the Commonwealth of Australia to remain current and integrated with the updated NHMRC consumption guidelines (2020). In order to ensure that guidelines remain relevant, the next set of guidelines should be updated in 2025, consistent with NHMRC recommendation that guidelines be updated every five years. These guidelines aim to provide up-to-date, evidence-based information to clinicians on available treatments for people with alcohol problems and are largely directed towards individual clinicians in practice, such as primary care physicians (general practitioners, nursing staff), specialist medical practitioners, psychologists and other counsellors, and other health professionals. Some chapters highlight service or system level issues that impact on clinicians and their patients. These include recommendations concerning Aboriginal and Torres Strait Islander peoples, culturally and linguistically diverse groups, stigma, and discrimination. Elsewhere, organisation capacity is implied, such as medical resources for withdrawal management where recommendations indicate use of medications. As all forms of treatment will not be readily available or suitable for all populations or settings, these guidelines may require interpretation and adaptation.
