Book excerpt: The blowout of the Macondo well on April 20, 2010, led to enormous consequences for the individuals involved in the drilling operations, and for their families. Eleven workers on the Deepwater Horizon drilling rig lost their lives and 16 others were seriously injured. There were also enormous consequences for the companies involved in the drilling operations, to the Gulf of Mexico environment, and to the economy of the region and beyond. The flow continued for nearly 3 months before the well could be completely killed, during which time, nearly 5 million barrels of oil spilled into the gulf. Macondo Well-Deepwater Horizon Blowout examines the causes of the blowout and provides a series of recommendations, for both the oil and gas industry and government regulators, intended to reduce the likelihood and impact of any future losses of well control during offshore drilling. According to this report, companies involved in offshore drilling should take a "system safety" approach to anticipating and managing possible dangers at every level of operation-from ensuring the integrity of wells to designing blowout preventers that function under all foreseeable conditions-in order to reduce the risk of another accident as catastrophic as the Deepwater Horizon explosion and oil spill. In addition, an enhanced regulatory approach should combine strong industry safety goals with mandatory oversight at critical points during drilling operations. Macondo Well-Deepwater Horizon Blowout discusses ultimate responsibility and accountability for well integrity and safety of offshore equipment, formal system safety education and training of personnel engaged in offshore drilling, and guidelines that should be established so that well designs incorporate protection against the various credible risks associated with the drilling and abandonment process. This book will be of interest to professionals in the oil and gas industry, government decision makers, environmental advocacy groups, and others who seek an understanding of the processes involved in order to ensure safety in undertakings of this nature.

To stay within, or close to Dome's budgetary estimate. 4. To complete the work within the pre-agreed study period. Due to the limits on money and time, it will be necessary to limit the alternates considered. Book excerpt: In FY 1990, Congress directed the Secretary of the Navy to commission a study by the National Academy of Sciences for the production of an integrated technology plan for the evolution of aircraft carriers in the first half of the twenty-first century. The House-Senate conferees emphasized "that the product of this study is to be a technology plan for the evolution of sea bases for the most efficient and economical accommodation of tactical air power in the first half of the twenty-first century". Based on this broad charter of evaluating sea bases, an examination of the floating ocean platform concept was included in the study. The floating ocean platform is a generic description of a large, relatively stationary or slowly mobile, platform that can be positioned in most areas of the ocean, and can serve a variety of purposes. The present report was the author's input to the study. It was based on technical analyses, literature reviews and surveys, and discussions/visits with the main groups and organizations involved in developing the floating ocean platform. All discussion material was unclassified, as are the contents of this report. All the external inputs and discussions, too numerous to mention, made this report possible, and are greatly appreciated. The first part of this report is the summary narrative that was submitted by the author to the Technology Group of the study. The second part is the viewgraphs that were presented to the Technology Group by the author on 12 February 1991. The third part is a selected bibliography of studies on the floating ocean platform over the past two decades, with over three thousand references identified.

(ii) Collect and analyse relevant standards, codes and regulations. (iii) Review the concrete caisson concepts developed by Dome for production, storage and loading platform in the Beaufort Sea and propose possible modifications. These concepts will be analysed according to various criteria which have a mutual influence. For simplicity we have described these criteria as: - structural and foundation; - functional; - construction and installation. (iv) Structural and foundation design review including: -environmental design forces (includes ice); - over-all stability calculation; - foundation calculation; - structural analysis of main members for both serviceability limit state (SLS) and ultimate limit state (ULS) conditions; - consideration of integrity, durability and protection. (v) Functional analysis review including: - assessment of the proposed arrangements of equipment and facilities to meet Dome's requirements for drilling, production, treatment and transfer; - assessment of the interrelationship of the various structural concepts and the functional requirements. (vi) Review of methods of construction, installation, and protection. The various proposed concepts will be analyzed from the point of view of their ease of construction, towing and installation: - fabrication site possibilities: water depth, sheltered areas, access, towing routes; - need for heavy construction equipment for prefabrication, deck mating, modules installations; - minimization of draft at tow-out from dry dock (partial construction, lightweight material, air cushion); - marine operation requirements: solid ballasting, water ballasting-deballasting, air cushion, grouting, retrieval of the structure; - need for site preparation, dredging, berm. (vii) Comparative cost estimating and scheduling including: - review of each concept in terms of quantities of material, equipment, operations, according to the analyses performed as per previous items (structure and foundation, functional needs, construction and installation facilities and methods); - review and update of the cost estimate and schedule for engineering, construction and installation.
