Great achievements have been made recently in neuroimaging techniques and computer technology for neuronavigation, from frameless, armless systems to robotic microscopes. Contained in the present volume are all the papers presented at the International Symposium on Computer-Assisted Neurosurgery and selected papers presented at the 6th Annual Meeting of the Japanese Society of Computers in Neurosurgery, which were held in Kobe, Japan, on January 24-26, 1997. This volume is a comprehensive description and review of current technical ad vancements in computer-assisted neurosurgery, with a special focus on advanced intraoperative neuroimaging, various neuronavigation system, robotic microscopes, and strategies for preoperative and intraoperative surgical planning using high-power workstations with three-dimensional software. We express our thanks to the contributors for their participation and cooperation, and to Springer-Verlag for personal and technical assistance in publishing this work. We sincerely hope that this volume will contribute to improving neurosurgical technology and outcomes.

This book is designed to serve as a comprehensive review of Computer-Assisted Surgery, covering the current status of both research and applications. CAS includes Virtual Preoperative Planning (VPP) and Intraoperative Virtual Navigation (IVN), which are a set of technologies used to measure oncological margins in 3-Dimensions (3D), to locate small intraosseous tumors and apply controlled resections preserving anatomical structures. During VPP, patient acquired multimodal images are processed and an interactive virtual scenario is created. This can then be used as a platform to measure oncological distances and preplan osteotomies in safe areas. IVN is a procedure which allows the execution of the VPP with a mean error of less than 3mm. For the student, medical doctors, research and development scientists or new researchers, the protocols are central to the performance of Computer-Assisted technologies.

Both the quantity and quality of the submissions allowed us to compose a volume of high quality papers, which we are sure will contribute to the further development of this exciting ?eld of research. As for the hard numbers, in total 338 submissions were received. Next to full papers, short communications were solicited for works in progress,hardware prototypes, and clinical case studies. Long papers were reviewed by three or four reviewers and short papers by two or three reviewers. The ?nal selection of papers was carried out by the Programme Board. Out of the 246 long papers, 36 were accepted for oral presentation and 100 as full posters. An additional 75 of the long papers, and 47 out of 92 short papers were accepted as short posters.

The goal of the procedure is decompression of the spinal nerve root by percutaneous removal of the nucleus pulposus under local anesthesia. Probably 20 % of all patients requiring lumbar disc surgery can be successfully treated by this method. During the past two years, percutaneous discectomy has spread rapidly, and it is now performed in most clinical departments engaged in spinal surgery. The first International Symposium on Percutaneous Lumbar Discectomy, held in Berlin in August 1988, covered all current procedures known as "percutaneous discectomy" and the entire range of percutaneous techniques, both clinical and experimental. Its publication is important because of the recency of this new surgical procedure, the outstanding experience of the speakers - including the Japanese, American, and European "pioneers" of the technique - and last but not least the gaps in the knowledge of physicians concerning this topic. This procedure opens up new perspectives in the surgical treatment of degenerative diseases of the lumbar spine.
