Book excerpt: "In a regionalized trauma system where patients may initially be assessed at a peripheral site, patient transfer represent one of the riskiest processes of trauma care and effective transfer mechanisms are required. Relatively few studies have looked at the trauma transfer process, especially in the province of Quebec. In a prospective study, we examined the timing and quality of trauma transfers arriving at a Level I trauma center in Montreal, from referral centers within island, regionally and rural Quebec for a 1 year period. Data from 387 patients were analyzed. The mean time for transfer for within the island of Montreal, regionally and Northern Quebec were respectively 648 (SE 85), 612 (SE 166) and 1376 (SE 167) minutes. On evaluation of aspects of critical care during transport, 5.7% needed an airway management change, 4% needed chest tubes, and 11% needed changes in circulation management. On survey of trauma leaders, 9.4% of transfers had potentially life-threatening issues during the transfer process. Further studies are needed to identify specific areas causing delay and risk factors for suboptimal critical care management during transport. Greater understanding of the transfer process will allow advocacy better for public health measures, transport teams and transport mechanisms. Computed tomography has become an integral part of the evaluation of a trauma patient for early and accurate detection of injuries however, determining which patients will benefit most of imaging have been more controversial. Though CT head and neck rules have been developed, no widely accepted guidelines have been developed for CT chest, CT abdomen or which patients require a whole body CT abdomen. A prospective study was performed over 1 year to evaluate the patterns of CT use at a Level 1 trauma center. 1852 scans were performed in 620 patients. 90% of these patients received a CT head, while rates of CT neck, chest and abdomen were 75, 66 and 67% respectively. The most commonly cited rational for CT head was anatomical findings whereas for other regions of the body, the most commonly cited rational was mechanism of injury. Scans were performed for anatomical or physiological findings are more often associated with positive findings on CT. CT scans were important in the decision making process. 45-67% of scans aided in the decision to discharge the patient. Further research is needed to develop a decision rule for which patients will require more extensive imaging such as whole body CT scanning.In another study also looking at the use of imaging in trauma, the patterns of screening for blunt cerebrovascular injury were assessed at our institution to determine whether patients were adequately screened according to BCVI guidelines. A retrospective chart review was performed for a 4-year period to identify patients who had injuries associated with BCVI. The incidence of BCVI at our institution was 1.3%. Screening for BCVI occurred consistently for patients with cervical, basal skull fractures but not LeFort or mandibular fractures. Multiple regression analysis showed basal skull fractures and cervical fractures to be independent predictors of BCVI with odds ratio of 2.01 and 8.45 respectively. Results suggest that guidelines for screening of BCVI in patients with mandibular and facial fractures are not consistently applied. A prospective study investigating the association between mandibular fractures, facial fractures and BCVI is needed to further define the relationship. " --

Book excerpt: The introduction of computed tomography in the diagnosis of pathological intracranial conditions has had considerable significance in cases of cranio cerebral injury. The decisive diagnostic advantage lies in the possibility of demonstrating both gross pathological change directly as well as secondary changes in normal brain structures. Computed tomography has proved its considerable worth, especially in evaluation of patients with craniocerebral injury and its sequelae. The capabilities of CT were quickly recognized and use of the technique spread rapidly. It is likely that CT will be available within a few years in all hospitals and clinics treating patients with craniocerebral injury. We believe it appropriate to present a detailed report on our experience with CT in 1800 cases of craniocerebral injury treated in the neurosurgical departments in Miinchen-GroBhadern and Berlin-Charlottenburg over a period of five years. Both acute posttraumatic complications and late sequelae are discussed extensively. A large number of illustrations is provided in order to facilitate the reader's introduction to CT diagnosis. The great interest in our conjoint study originally published in the German language, induced us to translate this book and to update the clinical material.
