Book excerpt: The development of mathematical methods for analyzing pathological speech signals is one of the best approaches to non-invasive diagnosis of laryngeal dysfunctions in patients with vocal fold disorders. The processing of disordered voice signals is a useful technique that provides a low-cost and time-efficient clinical tool for automatic detection of neurological and physical damages to the vocal folds. The application of these methods can help speech pathologists routinely monitor the functionality of patients' speech production system and provide them with appropriate therapies, medication or treatments. Moreover, these method can help detect the early symptoms and prevent the progress of some severe types of these diseases (e.g. laryngeal cancer) by tracking changes in the acoustical parameters of a patients' voice. In this book, a variety of signal processing methods are proposed, implemented and discussed with relevance to the diagnosis of different types of laryngeal diseases. The discussed methods include classical and novel signal processing approaches to the diagnosis of laryngeal diseases with specific focus on vocal fold disorders in patients with pathological speech.

When a microphone captures the voice, an acoustic signal can be produced and stored on a computer. The distinct signal pattern is an excellent indication of the quality of voice, and can be used to compare deviant voices with healthy voices. This study focuses on a computer analysis of laryngeal function-specifically on the acoustic analysis of vocal fold vibration in normal subjects and patients with specific voice disorders. The vocal pattern of each patient is analyzed using the MatLab computer program, producing a graph showing the unique vocal pattern for each patient. The acoustic recording of the vocal signal is observed, along with the analysis using fast Fourier Transform and short Fourier Transform. The patterns of many voice disorders are compared, including those in patients with vocal fold scarring and vocal fold cysts. A comparison is then made between the patterns of patients with a "normal" voice and those with the specified disorders. A brief introduction to the laryngeal image based analysis of glottal waveform is included, and compared with acoustic analysis results.

Vocal fold scarring can result from surgical excision of benign and malignant vocal fold lesions. The elicited inflammation and healing response is complex and dynamic. It involves immune and repair cells (e.g., neutrophils, macrophages, fibroblasts) and molecules (e.g., inflammatory cytokines, damage-associated molecular pattern molecules) to locate damaged tissue, eliminate the necrotic cells and debris, and produce extracellular matrix substances for eventual tissue repair. A mechanistic understanding of the vocal fold injury and repair process will help in identifying the effective treatments for iatrogenic vocal fold scarring from surgery.The overall goal of this thesis was to develop computational agent-based models (ABM) that could numerically simulate the dynamics of major cellular and molecular components involved in surgical vocal fold injury and repair. The first research goal was to phenotype and enumerate immune and repair cells in surgically injured rat vocal folds up to 4 weeks after surgical injury. Viable cells were isolated from harvested vocal folds and were stained for cell surface markers specific to neutrophils, macrophages, endothelial cells and fibroblasts. Multi-parametric flow cytometry was then conducted to count the number of cells for each cell population in the vocal fold samples. The second research goal was to calibrate and validate existing agent-based models of vocal fold injury and repair (VF-ABM) with the flow cytometry data. Global sensitive analysis using Random Forests were first employed to identify the most influential model parameters on the outputs of cell numbers. The Robust Parameter Estimation (ROPE) algorithm from the package of Statistical Parameter Optimization Tool for Python (SPOTPY) was then used to calibrate VF-ABM for early time points from Day 1 to Day 5 of the flow cytometry data. Subsequently, VF-ABM were run 100 times to generate 95% confidence intervals for model evaluation. VF-ABM predicted outputs were considered accurate if the empirical data points were within the 95% confidence intervals of simulated outputs. Cell population quantities were calculated for Day 7, Week 2 and Week 4 respectively against the empirical flow cytometry data. Predicted cell populations reached 100% accuracy for Day 7. Although VF-ABM generated qualitative trends of cell population comparable to those of Weeks 2 and 4, the corresponding empirical data values fell outside of the 95% confidence intervals. Results from the flow cytometry study provided quantitative and time-dependent data on major cell populations involved in surgical vocal fold injury. Results from the model verification informed the area of improvements for the next generation of VF-ABM. This thesis contributes to the development of a computational tool that would better inform clinicians in prescribing treatments for patients with voice disorders." --

The advances in digital signal processing and pattern recognition have motivated the research community to work on the development of computational tools to perform automatic analysis of speech. Most of the contributions on this topic are focused on sustained phonation of vowels and only consider recordings of one language. This thesis addresses two problems considering recordings of sustained phonations of vowels and continuous speech signals: (1) the automatic classification of Parkinson's patients vs. healthy speakers, and (2) the prediction of the neurological state of the patients according to the motor section of the Unified Parkinson's Disease Rating Scale (UPDRS). Recordings of three languages are considered: Spanish, German, and Czech. German and Czech data were provided by other researchers, and Spanish data were recorded in Medellin, Colombia, during the development of this work. Besides the classical approaches to assess pathological speech, a new method to model articulation deficits of Parkinson's patients is proposed. This new articulation modeling approach shows to be more accurate and robust than others to discriminate between Parkinson's patients and healthy speakers in the three considered languages.
