In fact, psychiatric patients are no more likely than patients in other medical specialities to go against the advice of their doctor. That said, it is easy to find instances where psychotropic medication is refused by the supposed beneficiary. The value of neuroleptic treatment in schizophrenia is now widely accepted. Failure to take such treatment is associated with relapse and relapse may endanger the patient and other people. Despite this, people with schizophrenia frequently fail to take their treatment. This study shows that one third can be expected to be non-compliant within two years of leaving a general adult psychiatry ward. It also looks at the reasons for this: the influence of drug side-effects is examined, as well as the impact of each patient's attitude to treatment and whether or not they have stopped taking prescribed medication in the past.

However, clinical and functional outcomes are still far less than optimal for patients, and have not improved in recent years despite the development of several new antipsychotics. Efficacy rates are further compromised by medication non-adherence, which has been reported to affect more than half of patients. In response to these issues, several non-pharmacological interventions have been developed for the treatment of schizophrenia, such as cognitive behavioral therapy, cognitive remediation, social cognition training and metacognitive approaches. Although these interventions have produced promising results, there is still much controversy regarding their usefulness and applicability in clinical practice. A major impeding factor for their dissemination is possibly a lack of sufficient evidence regarding their specific indications, mechanisms of action, adverse effects, but also practical issues concerning the interpretability of respective clinical studies, such as the choice of outcome variables and control of confounding factors. The present Research Topic includes original research articles and reviews addressing these issues.

The age of patients from 18 years. Disease duration at the time of the survey was at least 1 year. The study used: The scale of assessment of compliance with the treatment regimen (MARS, Thompson et al., 1999).Results:At the first hospitalization, according to the patients themselves, only 37 people were aware of the need for maintenance therapy (37%), realized but did not follow the recommendations of the attending physician - 51 people (51%), did not realize and did not follow the doctor's recommendations - 22 people (22% ). Within a year after the first hospitalization, 78 people (78%) were re-hospitalized (78%), of whom, against the background of non-compliance with drug therapy or partial non-compliance, 62 people (62%).Conclusion:Patients with schizophrenic spectrum disorders during the first year after the manifestation of the disease in most cases either did not comply with the regimen of drug therapy, or completely stopped taking the drugs, which is combined with a low level of awareness of the need for patients to continue therapy.
