Book excerpt: This study evaluated the use of the newest generation multiple auditory steady state responses (ASSRs) and auditory late responses (ALRs) in objective hearing threshold estimation in awake adults. A within-subject performance comparison between the 40 Hz ASSR and the ALR was conducted in 25 participants. Each participant was assessed monaurally using the Interacoustics Eclipse EP25. The test ear and order of testing were counterbalanced across participants. Stimuli centred at four test frequencies: 500, 1000, 2000 and 4000 Hz were presented via Eartone 3ATM insert earphones. The 40 Hz multiple ASSR used the narrowband (NB) CE-Chirp©, whilst the ALR used the long duration (80 ms) tone bursts. Note that for the ALR, 500 and 2000 Hz were prioritised, with the inclusion of 1000 and 4000 Hz if time and participant status permitted. The total test time for the 40 Hz ASSR to obtain a four-point audiogram was obtained by the time recorded on the software. The ALR test time was calculated in two forms: “actual” and “estimated”. The “actual” test time was calculated using the number of sweeps recorded for obtaining two frequencies (500 and 2000 Hz). The “estimated” time to obtain a four-point audiogram was calculated by doubling the “actual” time. Relative accuracy in estimating the behavioural audiogram was determined by comparing thresholds from each AEP measure. Book excerpt: The Auditory Steady State Response (ASSR) procedure has been established as a frequency specific, objective audiologic measure, which can provide reliable thresholds to within 10 dB of the behavioral thresholds. In order for ASSR to find its place in the existing framework of audiometric procedures, the full potential of the procedure needs to be explored. The aim of this study was to determine the accuracy of monotic ASSR in estimating hearing thresholds in a group of 15 normal hearing subjects and 15 hearing-impaired subjects. A comparative research design was implemented. Indicating that results obtained in the study was compared to relevant literature where dichotic multiple ASSR was implemented. This was done in order to ascertain ASSR's capabilities with regard to stimulus presentation methods. Monotic single ASSR predicted behavioural thresholds in the normal hearing subjects within an average of 24 dB across the frequency range (0.5, 1, 2 & 4 kHz). In the hearing-impaired group, ASSR thresholds more closely resembled behavioural thresholds, with an average difference of 18 dB, which is consistent with recent literature. The literature suggests that better prediction of behavioural thresholds will occur with greater degrees of hearing loss, due to recruitment. The focus in this group also centered on the accurate prediction of the configuration of the hearing loss. It was found that ASSR could reasonably accurately predict the configuration of the hearing loss. In the last instance, monotic single and dichotic multiple ASSR were compared with regard to threshold estimation and prediction of configuration of the hearing loss in the hearing-impaired group. Little difference was reported between the two techniques with regard to the estimation of thresholds in both the normal hearing and hearing impaired groups. In conclusion it was established that monotic ASSR could predict behavioural thresholds of varying degrees and configurations of hearing loss in normal and hearing-impaired subjects with a reasonable amount of accuracy. At this stage, however, more research is required to establish the clinical validity of the procedure, before it is routinely included within an objective test battery.

Although new stimuli are being proposed and clinically implemented, there are no data to indicate whether response interactions would be adversely affected by their use. This study investigated the effects of three different stimuli (AM, AM/FM and AM2) at two different intensities (60 dB HL and 80 dB HL) on response amplitudes and interactions in normal-hearing adults. Stimuli were generated by the Rotman MultiMASTER research system and presented via air conduction through EAR-3A insert earphones. Carrier frequencies of 0.5, 1, 2, and 4 kHz were 80-Hz modulated in three conditions: individually (monotic single; MS), simultaneously in one ear (monotic multiple; MM), and simultaneously in both ears (dichotic multiple; DM). It was predicted that stimuli with broader spectra would result in greater amplitudes. This was demonstrated in the MS condition by the AM/FM stimulus, which evoked responses significantly larger than those to both AM and AM2 stimuli at all frequencies except 0.5 kHz at 60 and 80 dB HL. In the multiple (MM and DM) conditions, response amplitudes to AM2 were significantly larger than AM and AM/FM response amplitudes at both intensities. It was also predicted that more interactions would be found when using stimuli with broader spectra, even at moderate intensities. This was illustrated by the drop in amplitude by the AM/FM stimulus in the multiple conditions versus in the single condition, even at 60 dB HL. Relative efficiency values in the multiple conditions were never less than that found in the single condition at 60 dB HL; at 80 dB HL, the majority (83%) of comparisons were more efficient in the multiple conditions than the single condition. Based on these results, the optimal stimulus to use appears to be dependent on the chosen condition. In the single condition, AM/FM stimuli result in the largest response amplitud.
