Book excerpt: Approximately 80% of individuals post stroke experience some form of mobility impairment associated with hemiparesis. Impaired mobility leads to reduced independence and quality of life. Gait rehabilitation is often the primary goal of individuals post stroke. As clinical understanding of human locomotion has evolved, there has been a shift in recent years from more traditional gait rehabilitation methods towards the inclusion of sensory stimuli such as rhythmic auditory stimulation (RAS) and visual feedback (VFB). There is however limited knowledge about the comparative effects of stimulation to the visual and auditory cortex during gait rehabilitation training. Purpose: The aim of this study was to compare the effects of visual feedback and rhythmic auditory stimulation on walking performance in individuals with hemiparetic gait. Design: A total of 5 participants (age 61± 6.928) with hemiparesis completed 5 weeks of treadmill walking incorporating visual feedback and 5 weeks of treadmill walking incorporating rhythmic auditory stimulation. Participants first completed 5 weeks of VFB training using the BIODEX Gait Trainer II® treadmill followed by a 5 week detraining period and then a final 5 weeks of treadmill walking with RAS via digital metronome. Gait analysis was performed using the VICON 3D motion analysis system. Participants had their gait analyzed using the VICON plug-ingait model before and after each 5 week training intervention. Gait outcomes included stride length, step length, walking speed, cadence and lower extremity joint excursion at the hip, knee and ankle. Book excerpt: Gait parameters and synchronization capabilities were examined following a one-day experimental implementation of Rhythmic Auditory Stimulation (RAS) for adults with severe traumatic brain injury (TBI). Eight individuals (N=8) who had experienced severe TBI participated in this study. Participants ambulated four 10-yard trials across a plastic runner along a flat surface with paint applied to their shoes (via the footprint analysis gait assessment). Parameters including cadence, velocity, stride, step lengths, and symmetry were calculated through footprint and videotapes analysis. All participants ambulated under the following conditions: baseline with no music, RAS at resonant frequency, RAS with a 10% frequency modulation increase, and removal of RAS. Quantitative data were reported through raw scores as well as percentage change between trials. Results of quantitative data and gait analysis revealed that at least 7 out of 8 participants showed capacity to synchronize to an external rhythmic auditory cue for portions of a trial, respond to the frequency modulation, and/or reproduce a given frequency cue once rhythm was removed. Gait kinematics appeared optimal for most participants within the resonant frequency pass (trial 2), especially including decreases in asymmetry.

With the advance of modern neuroimaging techniques during the past decades, we are now beginning to understand better what goes on in the healthy brain when we hear, play, think, and feel music and how the structure and function of the brain can change as a result of musical training and expertise. For more than a century, music has also been studied in the field of neurology where the focus has mostly been on musical deficits and symptoms caused by neurological illness (e.g., amusia, musicogenic epilepsy) or on occupational diseases of professional musicians (e.g., focal dystonia, hearing loss). Recently, however, there has been increasing interest and progress also in adopting music as a therapeutic tool in neurological rehabilitation, and many novel music-based rehabilitation methods have been developed to facilitate motor, cognitive, emotional, and social functioning of infants, children and adults suffering from a debilitating neurological illness or disorder. Traditionally, the fields of music neuroscience and music therapy have progressed rather independently, but they are now beginning to integrate and merge in clinical neurology, providing novel and important information about how music is processed in the damaged or abnormal brain, how structural and functional recovery of the brain can be enhanced by music-based rehabilitation methods, and what neural mechanisms underlie the therapeutic effects of music. Ideally, this information can be used to better understand how and why music works in rehabilitation and to develop more effective music-based applications that can be targeted and tailored towards individual rehabilitation needs. The aim of this Research Topic is to bring together research across multiple disciplines with a special focus on music, brain, and neurological rehabilitation. We encourage researchers working in the field to submit a paper presenting either original empirical research, novel theoretical or conceptual perspectives, a review, or methodological advances related to following two core topics: 1) how are musical skills and attributes (e.g., perceiving music, experiencing music emotionally, playing or singing) affected by a developmental or acquired neurological illness or disorder (for example, stroke, aphasia, brain injury, Alzheimer’s disease, Parkinson’s disease, autism, ADHD, dyslexia, focal dystonia, or tinnitus) and 2) what is the applicability, effectiveness, and mechanisms of music-based rehabilitation methods for persons with a neurological illness or disorder? Research methodology can include behavioural, physiological and/or neuroimaging techniques, and studies can be either clinical group studies or case studies (studies of healthy subjects are applicable only if their findings have clear clinical implications).

This Research Topic will bring together neuroscientists interested in brain plasticity and the effects of training, psychologists working with adults as well as with normally developing children and children with learning disabilities as well as education researchers directly confronted with the efficiency of education programs.
