Néanmoins, au sein des services d'urgences, les équipes médicales sont constituées par des médecins de formation initiale et de niveau de compétence hétérogène. Nous avons proposé de comparer la performance diagnostique de deux catégories de médecins des urgences. La première était représentée par le Praticien Hospitalier (PH) véritable spécialiste en médecine d'urgence. La seconde catégorie était plus hétérogène quant à sa formation et à son expérience, mais le plus souvent, en cours de Capacité de Médecine d'Urgence (assistants, praticiens attachés...) ou disposant d'une moins grande expérience de terrain. Nous avons proposé une méthodologie originale, prospective et randomisée. La performance diagnostique était évaluée à partir de cinq symptômes recours (douleur thoracique, douleur abdominale, dyspnée, trouble de conscience et fièvre). La comparaison du diagnostic initial des urgences avec le diagnostic final d'hospitalisation constituait le critère principal de jugement. Les critères secondaires de jugement était, d'une part, le niveau de conviction pour le diagnostic retenue et, d'autre part, le nombre d'examens complémentaires prescrits utiles au diagnostic. A ce stade du travail, nous n'avons réalisé qu'une étude de faisabilité, n'autorisant pas une analyse statistique des résultats. En revanche, les conditions requises pour le bon déroulement de l'étude définitive ont été mises en évidence : présence indispensable d'un infirmier organisateur de l'accueil, adhésion forte des personnels médicaux et paramédicaux, et nécessité d'une étude multicentrique. Dans l'hypothèse d'une meilleure performance diagnostique d'un des deux types de praticien, il en découlerait des conséquences d'ordre organisationnel : rôle de coordonnateur de l'accueil et de la gestion des flux de patients, rôle d'expert au sein du service, et enfin un rôle d'interlocuteur privilégié avec les autres structures hospitalières ou extrahospitalières
