Book excerpt: This publication is intended as a guide for the clinical commissioning of radiotherapy treatment planning systems (RTPSs) and provides a simple protocol for these tasks. The procedures for clinical commissioning tests cover typical treatment techniques used in the radiotherapy hospitals and are based on the use of a specific phantom. The purpose of this testing is twofold. Firstly, the tests will provide an educational opportunity for the user to become familiar with the operation of the RTPS. Secondly, the tests will demonstrate to the user that the logistic chain starting from CT scanning, anatomic modelling, treatment planning and MU calculation is operable and leads to the desired results with sufficient accuracy.

Record and verify systems (RVSs) were developed to reduce the risk of treatment errors in radiation oncology. These have recently evolved into complete radiotherapy information management systems that interface with imaging systems, treatment planning computers, and treatment delivery systems. To function as intended, RVSs must be subject to a comprehensive quality assurance (QA) program. This publication provides practical guidelines for a comprehensive QA programme and its implementation. It describes the QA program, including acceptance tests and the commissioning process that should be used in conjunction with the installation of a new RVS. It is also highlighted that some of the tests performed at installation must be repeated regularly as part of the periodic quality control checks.

Book excerpt: Treatment planning system (TPS) commissioning is time consuming, labor intensive and error prone. Guidelines exist for commissioning procedures yet media and reports expose harmful dosimetric inaccuracies due to human error. This dissertation intended to develop a model for reducing errors and improving efficiency in the TPS commissioning process. This project aimed to 1) evaluate the level of risk for current published guidelines for TPS commissioning and determine where errors could be reduced 2) design an automated commissioning test suite (ACTS) that incorporates recommended tests from multiple reports and task groups 3) evaluate the level of risk for the proposed automated commissioning process (ACP) that uses automation and standards: preloaded/pre-modeled beam data, preloaded digital phantoms, ACTS, application program interface (API), validation measurements using an Electronic Portal Imaging Device (EPID) for improved benchmarking to cloud storage for access to data and ongoing QA 4) compare associated risk between manual and automated TPS commissioning procedures 5) create proof of concept script and conduct testing. The data shows that there are fewer failure modes and lower associated risk when TPS commissioning tests are automated. The presented work suggests that errors and risks may be reduced by applying standardization and automation to the TPS commissioning process.
